Palliative Community Programs

Carefor’s Palliative Community Programs offer people dealing with a terminal illness support in a variety ways. They help people connect with others and resources to manage the emotional and physical reality of this stage of their journey.

  • Palliative Day Program

    The Palliative Day Program is supported by Carefor Hospice Cornwall & provides a welcoming, friendly environment that gives clients an opportunity to share with others going through similar life experiences.

    Activities of the support group may include discussions, reflexology, therapeutic touch, music, massage, pet therapies, reading, games, puzzles, and guest speakers. Support services and a quiet room are also available throughout the client’s visit. A hot lunch and delicious snacks will be available throughout the day.

  • Volunteer Visiting Program

    The Palliative Care Volunteer Visiting Program aims to ease the burden of dealing with a terminal illness. Our dedicated and helpful volunteers support clients and caregivers so they have the time and energy to enjoy the highest quality of life each day. Volunteers also help alleviate physical, emotional and spiritual suffering.

    Our services include caregiver relief, outings, encouraging and supportive friendships where similar interests and activities are shared, support in times of crisis, and more.

  • Caregiver Support

    The support group exists to provide social and emotional support to individuals with a terminal illness and their families, therefore allowing them to remain in their community. The support group compliments services already provided for terminally ill clients and their families residing in Cornwall, Stormont, Dundas & Glengarry. Anyone can make a referral: the client, caregiver, health care professional, clergy or a friend.

Volunteer at Hospice

Volunteers are an integral and valued part of our team at Carefor Hospice Cornwall.

We offer specialized training in all areas. All volunteers are required to have a valid police check on file to work within our organization. They also must take part in a Palliative Care Course, given by a registered health care professional. This course covers all aspects of end-of-life care and how it pertains to volunteering at Hospice.

Patient & Family Support

Support through your palliative journey

The Hospice team helps you and your loved one by providing a reassuring presence to allow you to spend time together as this part of our journey.

The Hospice team:
  • Listens, assesses and evaluates any concerns or perceptions the resident may have, encouraging the resident to express his or her wishes, needs and fears.
  • Advocates on their behalf, helping everyone to work through and understand many of the changes that occur as the resident’s health declines.
  • Assists the resident to develop a plan of care including psychological, emotional and spiritual needs and any and all community resources necessary to make informed decisions about such things as: funeral arrangements, financial or legal matters or survivor benefits, ensuring that no one is excluded.
  • Helps to prepare residents, families and other significant others with knowledge of what they can generally anticipate about the dying process. Most important is talking about and normalizing the kaleidoscope of emotions that people have during this difficult time.
  • Assists family and close friends who are experiencing the waves of emotional distress just prior to or after their loved one has died. The Patient & Family Support.
  • Coordinator makes follow-up calls to some families to see how a spouse or other loved one is coping after the death and offers short-term counselling for those in need after their loss.
  • Provides education to volunteers and staff on such topics as compassion, fatigue and healthy caregiving.

Bereavement & Grief Support Groups

Helping families heal

We offer regular support groups as well as a 6-week closed group session for Hospice family members and caregivers who have lost a loved one.

Each session is administered by Bereavement Facilitators. One-on-one meetings can be arranged at your request with the Family & Patient Support Coordinator.

  • Regular Support Groups

    Where: Hospice Cornwall (Advanced registration is required)

    When: Tuesdays from 6:00-7:30 PM, and Thursdays from 2:00-3:30 PM

    Our regular support groups offer clients a safe and comfortable environment with open-group-style conversation. Group discussions include a variety of topics for those who have lost a loved one. Light refreshments will be included.

  • 6-Week Closed Group

    Where: Hospice Cornwall (Advanced registration is required)

    Time: Thursday mornings over a 6-week period

    Our closed group meetings will be offered throughout the year in a structured format with a maximum of 8 participants. Group discussions include a variety of topics for those who have lost a loved one. Light refreshments will be included.

In Community Services

Carefor offers home care community services and retirement homes for people in Eastern Ontario

Services in your community to help you stay at home

You want to be able to continue to live in your home and Carefor’s community services help you do just that. We offer a variety of services which help you stay connected.

  • Bereavement Support

    Staff and volunteers support individuals, family, and friends following the death of a loved one, individually or in a group setting.

  • Family Resource Centre

    Staff and volunteers provide education and resources to help clients and families find information (for example, internet access to resources, library materials, workshops, and webinars) and one-on-one support services.

  • Personal Support Services

    If you need some help with your care, our trained Personal Support Workers can help.

    Some of their services include:

    • Personal Care: bathing, grooming, dressing, exercise and more.
    • Respite Care: Relief for caregivers, companionship and life enrichment activities.
    • Home Management: Light housekeeping and meal preparation

    Service is available 24 hours a day, seven days a week in blocks of time from 1 to 24 hours.

    There is a fee for service.

Carling Adult Day Program

Come for the care, stay for the fun.

The Carling Day Program gives people living with dementia a chance to enjoy a fun day out with friends in a safe and supportive setting, while giving their loved ones a well-deserved break.

Information, supportive counseling and referrals to other community services and programs are also available.

Program details

When is it: Monday to Friday from 9:00 am – 3:00 pm

Cost: $25.00 per day.

How can I join: Program referrals are made by Home and Community Care Support Services Champlain (HCCSS), as well as health care professionals and other Community Support Services and programs.

Inquiries are always welcome by calling the program directly.

Transportation

Cost: We can arrange transportation for participants living within the program’s boundaries for $16.00 (return trip).

Pick up: Participants are usually picked up between 8:00 and 9:00am

Drop off: Between 2:45 and 3:30pm.

Staff can also assist with accessing community transportation services or para Transpo. A family member or designate must be there to meet the client at the end of the day.

Aging at Home

Age at home with the utmost care.

The Aging at Home Program serves seniors with disabilities, providing personal support and essential homemaking services. Some of the services offered could include assistance with personal grooming and hygiene, bathing and washing, toileting, rinsing and retiring, dressing and undressing, transferring, meal preparation, laundry and light housekeeping.

In home Respite

Respite support is just as important for you as it is for your loved one.

The Respite Program is a home care support service which allows you to schedule temporary relief from your role as a caregiver. This offers safe support for your loved one and helps alleviate the stress and strain that can be placed on relationships during day-to-day care. Services could include personal care, homemaking, and supervision.

Community Supportive Housing

Personal support in a safe, residential setting.

Community Supportive Housing is supported by a partnership between Carefor and Cornwall & Area Housing Corporation. The program serves adults with disabilities by providing personal support services and essential housekeeping services in a safe environment. Attendant care staff are on-site 24/7, and provide non medical support care.

Assisted Living Services

If you need some extra help at home.

A Home and Community Care Support Services funded program, Carefor’s Assisted Living Services are community-based health care services that provide eligible seniors living in Pembroke with the assistance they need to allow you to continue living on your own for as long as possible.

There is no cost for these services.  Spaces are limited and there may be a waitlist.

Going Home Program

The Going Home Program helps seniors remain safely at home after leaving the hospital.

The program offers a basket of community support services, including frozen and hot meals, transportation, homemaking and personal support services. Services are free of charge within the first 10 business days when leaving the hospital. 

How do I get service?
  • Assessment

    Clients are assessed and referred to the Going Home Program by the Hospital Discharge Planner and/or by the Geriatric Emergency Management (GEM) Nurse in the Emergency Departments of our Champlain District hospitals.

  • Eligibility
    • Age: 65 years of age and older.
    • Mobility aligns with Champlain Community Transportation Collaborative criteria.
    • Services will not be duplicated either through Home and Community Care, new or existing services in the community, or paid services.
    • Clients living in Retirement Homes are eligible for transportation services only.
    • Clients with contact precautions are eligible dependent on the availability of PPE for client and driver (if necessary), and referral includes a description of the precaution. Intake reserves the ability to refuse the referral if deemed unable to safely service the client.
