Job Description
  • What You Will Do:
    • Acts within the unregulated worker’s role when making observations regarding the clients health status, condition and situation
    • Uses information in the care plan, computer memos, and staff communication log when making observations, and seeks out guidance and information from the nurse, manager or other relevant staff members as required
    • Reports all changes in client condition/situation in accordance with Carefor policies and procedures
    • Contributes to the development and revision of client’s service plans in collaboration with the nurse
    • Sets service goals in collaboration with the client, the nurse and the manager
    • Any changes to the care plan must be implemented by the manager or nurse for safety, liability and financial reasons
    • Providing personal care  which includes assistance with all aspects of activities of daily living bathing, mouth and skin care, dressing and grooming, incontinent care, assistance in walking and transfers, climbing or descending stairs, feeding, exercises and bedside care
    • Performing delegated tasks (e.g. applying prescription ointment) where required, under the direction of the supervisor or a registered Staff.
    • Monitoring and maintaining resident and household safety (report to supervisor)
    • Recognizing the social and emotional needs of client and dealing with them effectively (report to supervisor)
    • Meal set-up or planning and preparing nutritious meals including special diets
    • Light housekeeping/cleaning (including bedrooms, bathrooms, floors, windows and other surfaces)
    • Laundering of clothes and linens (wash, fold, put away), including occasional ironing and essential mending of clothing; laundry for incontinence
    • Accompanying clients on outings or to appointments as necessary
    • Setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment
    • Completing assignments in the assigned length of time
    • Plan, lead and participate in various programs of meaningful one-on-one and small group activities geared to the guests’ abilities and preferences
    • Help create and maintain a peaceful,’ normalized,’ home-like environment for guests
    • Assist with recreation and personal interest activities for the clients
    • Ensure that the door remained locked at all times
    • Assisting clients to keep their rooms clean and tidy and making beds
    • Providing medication assistance to clients and signs for medications given
    • Making appointments for clients and accompanies them when necessary
    • Encouraging and promoting resident independence
    • Treating residents with dignity and respect
  • Qualifications
    • Secondary school education preferred (Grade 12)
    • Personal Support Worker Certificate, Home Support Worker Certificate (Level II or III), Health Care Aide
    • Current Standard First Aid
    • Current CPR – Level C
    • Dementia education/courses as asset

The Opportunity: Dietitian – Part-time Permanent

The Dietitian is responsible for the provision and management of cost effective Dietetic care to individuals and/or families in the community. Care is provided within the framework of quality & risk management and the standards and scope of practice set by Carefor and the provincial regulatory body (CDO) for the Dietitian. Job Description
  • What You Will Do:
    • Schedules service providers for client visits in areas assigned by manager
    • Provides work schedules to staff that meet client needs and maximize continuity of care.
    • Keeps up to date with daily changes such as staff availability, changes in client service
    • authorizations and/or care plans
    • Ensures client and service provider information is accurate and up to date in the scheduling database
    • Provides timely information, through the use of communication technology (telephone, fax, e-mail, voicemail), to field staff, services providers, co-workers, Managers and Supervisors, clients and customers
    • Verifies visit confirmations from field staff and processes in a timely manner to be processed by the billing department
    • Verifies payroll information for field staff in a timely manner ensuring accuracy of hours, mileage, stats and premiums.
    • Contacts LHIN regarding discrepancies in frequency, authorization for extra visits or not seen/ not found visits, extensions to planned review dates
    • Works collaboratively as part of a team willing to assist others and mentor new staff
    • Contributes to the principles of customer-focused service and continuous quality improvement
    • Functions and completes all duties in compliance with Occupational Health and Safety legislation, regulations, and Carefor policies, procedures, and standards
  • Qualifications

    Education:

    • Degrees/Certificates

Job Description
  • What You Will Do:
    • Provides complete, relevant assessment of the client’s functional abilities, general condition and activities of daily living following pre-set procedures
    • Implements the care plan based on the nursing assessment in collaboration with the physician, PSW’s and manager and review quarterly
    • Implements nursing strategies or interventions according to the established care plan
    • Performs technical nursing skills at a safe level in accordance with scope of practice, nursing principles and Carefor policies and procedures
    • Utilizes appropriate interviewing and communication skills to obtain information from the client/family
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor standards (by exception)
    • Evaluates client’s responses to care plan for the achievement of realistic client-centered outcomes; revises plan as required
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principles and established teaching materials to provide health teaching to clients, unregulated care providers and volunteers
    • Reports and discusses client’s status on a regular basis with other members of the health care team, including physician when necessary
    • Works closely with the care team (including psws, cooks, recreation/activity coordinator and/or volunteers), providing them with direction, teaching and support
    • Orders and checks new medications from the pharmacy and lists the medications on the MAR sheets
    • Admissions and Discharges of clients as required
    • Keep up to date on changes in the field of dementia care
    • Familiarity with the home’s fire, disaster and evacuation plans
    • Participate in providing personal care, activities of daily living, preparing and serving meals & snacks, light housekeeping, recreational/personal interest activities as necessary.

    • Oversee marketing, brand, and communications strategies to strengthen organizational reputation and stakeholder engagement.

    The Vice President serves as an executive steward of enterprise risk management and organizational resilience.

    Key Accountabilities

    • Lead the organization’s Enterprise Risk Management (ERM) framework, including identification, mitigation, monitoring, and reporting of strategic, operational, financial, human capital, and reputational risks.
    • Ensure alignment between risk management, strategic planning, and continuous improvement priorities.
    • Oversee corporate policy frameworks and ongoing policy development, internal controls, and compliance processes.
    • Support regulatory compliance across all accountable domains.
    • Provide regular risk reporting to the Executive Leadership Team and Board committees.
    • Model values-based, inclusive, and transparent leadership.
    • Foster a culture of accountability and innovation.

    Financial and Other

    • Leads the development of the annual operating budget for the assigned portfolios and is accountable for financial results of their departments
    • Provides input and expert judgment regarding expenditures and resources needed for the programs within their portfolio
    • Regularly monitors expenditures and revenues against the budget and adjusts course as required to stay within budget targets
    • Required to travel locally within Carefor’s service geography on a regular basis
  • Qualifications

    Education & Credentials

    • A university degree in health, public policy and strategy, business, commerce, human resource management, or a related field.
    • Master’s degree in Health Administration (MHA) or Masters in Business Administration (MBA) (or equivalent)
    • LEAN Healthcare Certificate and/or Project Management certification (e.g., PMP) or equivalent demonstrated experience

    Experience and Qualifications

    • Minimum 10 years of progressive senior leadership experience, with executive-level accountability
    • Demonstrated experience in healthcare organizations within Ontario’s regulated environment
    • Demonstrated success in driving enterprise-wide initiatives focused on enhancing organizational culture and continuous improvement
    • Experience in strategy development, business growth, and system partnerships
    • Strong exposure to corporate risk, governance, and board reporting
    • Experience working in unionized healthcare environments is strongly preferred

  • What You Will Do:

    Client-centred support and assistance with activities of daily living (ADLs), personal care and other care needs assigned. Duties will be carried out under the formal delegation of a registered nursing professional, in accordance with organizational policies, applicable legislation, and within the scope of training and competence. Responsibilities may include, but are not limited to the following:

    • Personal Care & ADLs: Assist clients with bathing, grooming, dressing, toileting, mobility, and safe transfers (including use of lifts and mobility aids).
    • Health Monitoring: Observe and report changes in client condition to the supervising nurse or care team.
    • Medication Assistance: Provide medication reminders or assistance as per care plan.
    • Environmental Support: Perform light housekeeping, laundry, and meal preparation to maintain a safe and clean environment.
    • Social & Emotional Support: Engage clients in conversation and activities to reduce isolation and promote well-being.
    • Documentation & Communication: Accurately document provided care and communicate effectively with clients, families, and team members.
    • Safety & Infection Control: Follow Personal Protective Equipment (PPE) protocols, infection prevention practices, and health and safety regulations.
    • Team Collaboration: Work collaboratively with nurses, supervisors, and other health care professionals to ensure continuity of care.
    • Specialized care: provide basic ostomy and catheter care; assist with end of life care and respite care where appropriate

Carefor is launching the Caregiver Speaker Series—an educational and inspirational initiative designed to support those caring for loved ones. Funded by Rexall, this six-part series will be hosted across Carefor’s centres in Eastern Ontario, at no cost to attendees.

Each session will feature Carefor’s frontline experts—occupational therapists, nurses, personal support workers, and social workers—sharing practical strategies for dementia care, safe mobility, and navigating community resources. The series offers caregivers not only knowledge, but affirmation and connection.

Topics will be decided based on consultation with each Centre team. They are experts in their communities and will know or may suggest topics that will create the most interest.

With 71 percent of Canadian caregivers reporting increased burnout since the pandemic, this program provides a vital space for learning, rest, and renewal. While caregivers attend, Carefor will provide on-site support for their loved ones, ensuring peace of mind and uninterrupted care.

The Opportunity: Registered Practical Nurse- Permanent

Reporting to the Manager of Allied Health Services, the Registered Practical Nurse, atHome Programs is responsible for the provision and management of quality nursing care to clients transitioning out of hospital as part of the Carefor’s atHome bundled care programs. The Registered Practical Nurse atHome programs is responsible for managing the wrap-around care that is coordinated and delivered to clients admitted to the program, with support from members of the care team. The Registered Practical Nurse will be completing InterRAI-HC assessments, participating in the development of care plans, engaging with other members of the care team (e.g., social workers, PTs, OTs, PTA/OTA, nutritional specialists, and speech language pathologists) and delegating tasks to personal support workers assigned to deliver care. Of note, the Registered Practical Nurse will utilize InterRAI-HC assessment findings to proactively develop care plans supporting interventions aimed at minimizing unnecessary emergency department visits by maintaining clients in their home with the necessary support.

The Registered Practical Nurse is also responsible for providing clinical support, identifying and following up with PSW job performance issues, participating in client case conferences, offering ongoing training and assessments, and providing feedback on performance.
As a resource to the atHome Program, the Registered Practical Nurse will support the care team and clients in providing safe, client-centered care by modeling practice standards, defining care processes and procedures, and collaborating in the design of personal support workers’ education and training.
Striving for excellence in quality client care, the position supports the implementation of evidence-based professional practice projects, programs, and initiatives for Carefor’s atHome Program(s).

Job Description
  • What You Will Do:
    • Supports the care management and home care assessment responsibilities of clients transitioning out of hospital within the atHome Program;
    • Delivers nursing care to clients within the atHome Program;
    • Acts as a resource to the atHome Program care team, providing coaching, education, and instruction to encourage individual and operational performance objectives. Encourages and models evidence-informed practice;
    • Supports the performance management process with assigned staff and engages with appropriate managers regarding any performance issues;
    • Contributes to human resources management including employee relations, health and safety, continuing education, performance development, recognition of staff and orientation, as required;
    • Assists the Manager in identifying areas for recruitment, anticipates and monitors staffing needs and initiates appropriate action to obtain/maintain an adequate compliment staff for the atHome program;
    • Investigates and evaluates client and staff concerns/complaints and implements appropriate resolutions, as required

    Operational And Organizational Responsibilities

    • Leads by example and promotes continuous quality improvement;
    • Understands, promotes, and communicates the value, purpose, and goals of the atHome Program;
    • Working with the Allied Health Services Manager in the evaluation and revision of atHome Program policies and procedures, based on changes in practice and requirements from funder or partner organization;
    • Ensures that the quality of client care delivery is consistent with Carefor’s mission, vision, values and strategic plan;
    • Meets the contractual terms set forth by external/funding partners and maintains communication with these partners as needed;
    • Assists with operation of the program; provision of direct client care as required;
    • Collaborate to address quality, safety and risk management issues, including the creation of professional development and performance improvement plans to address shortfalls;
    • Contributes to the improvement of client outcomes (quality of life and quality of care) based on best available evidence and efficient utilization of resources;
    • Contributes to business development initiatives, as required;
    • Support the atHome Program team through regular meetings, orientation, mentorship, consultation, and guidance with complex clinical situations. Participate in internal and external case conferences or meetings as required; and
    • Operates within as well as monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures.

    Working Conditions

    • Combined regular office environment with some flexibility of remote work.
    • A vehicle for travel within the Ottawa region to support Nursing staff working in various community settings (e.g. individual homes, retirement homes, Carefor clinics) will be required.
    • Ability to work flexible hours to support client needs may be required.
  • Qualifications
    • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario; required.
    • Three (3) years recent experience of working within the adult home health care sector with related experience to support the provision of restorative care to older adults.
    • Knowledge of the related regulatory and legislative requirements within Ontario; required.
    • Certification in clinical specialties and chronic disease management (e.g., GPNC(C) Gerontology Practical Nurse Certified (C) Canada or CMSN(C) Medical-Surgical Nursing (C) Canada); an asset.
    • Ability to promote and facilitate evidence-based practice within a community setting with a demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.
    • Superior communication skills and ability to communicate effectively to direct and motivate others while maintaining a persuasive and credible presentation style at all levels of the organization.
    • Demonstrated effective analytical, and problem-resolution skills.
    • Excellent organizational, planning and time management skills.
    • Proficiency in using Microsoft Office applications (e.g., Outlook, Word, Excel, etc.).
