Job Description
  • What You Will Do:

    As a Registered Practical Nurse in Carefor’s Visit Nursing Program, you will make a difference in the lives of your clients in the community by utilizing your excellent communication skills to obtain relevant information from your clients and their families. You will implement nursing strategies or interventions according to the established care plan in consultation with the client and/or caregiver and the health care team. You will utilize adult learning principals and teaching materials to provide guidance to clients and their caregivers.   • What You Will Do:
    • Provides complete, relevant assessment of the client utilizing pre-set procedures
    • Develops a care plan with the client and family based on the nursing assessment, incorporating discharge planning

  • What You Will Do:

    KEY RESPONSIBILITIES

    Clinical Practice

    The RPN provides service consistent with the standards established by Carefor and CNO. The nursing process is utilized: assessment, planning, implementation and evaluation.

    • Provides complete, relevant assessment of the client’s functional abilities, general condition and activities of daily living following pre-set procedures
    • Implements the care plan based on the nursing assessment in collaboration with the physician, PSW’s and supervisor and review quarterly
    • Implements nursing strategies or interventions according to the established care plan
    • Performs technical nursing skills at a safe level in accordance with scope of practice, nursing principles and Carefor policies and procedures
    • Utilizes appropriate interviewing and communication skills to obtain information from the client/family
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor standards (by exception)
    • Evaluates client’s responses to care plan for the achievement of realistic client-centered outcomes; revises plan as required
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principles and established teaching materials to provide health teaching to clients, unregulated care providers and volunteers
    • Reports and discusses client’s status on a regular basis with other members of the health care team, including physician when necessary
    • Works closely with the care team (including Personal Support Workers (PSWs), cooks, recreation/activity coordinator and/or volunteers), providing them with direction, teaching and support
    • Orders and checks new medications from the pharmacy and lists the medications on the MAR sheets
    • Medication administration to clients as per CNO standards
    • Admissions and discharges of clients as required
    • Keep up to date on changes in the field of dementia care
    • Familiarity with the home’s fire, disaster and evacuation plans
    • Participate in providing personal care, activities of daily living, preparing and serving meals and snacks, light housekeeping, recreational/personal interest activities, as necessary.

     

    Leadership

    • Works collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for fellow nurses, PSWs, volunteers, and students.
    • Shares knowledge, assisting clients to make informed choices in determining health and service needs
    • Influences policy review and development in health care and standards of nursing practice through involvement in Carefor, professional organizations and/or community organizations
    • Help create and maintain a peaceful and home-like environment for guests

     

    Quality & Risk Management

    • Practices in accordance with the quality and risk management framework of Carefor
    • Articulates knowledge of components of quality and risk management
    • Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
    • Seeks input from clients and their family (POA) to assure that service meets expectations
    • Functions as an effective team member
    • Participates in decision-making and follows up to promote continuous quality improvement

    • Functions in accordance with the Standards of Nursing Practice from the CNO

    Communication

    • Receives direction and assignments from the supervisor and/or manager by phone or in-person
    • Observes the client’s pattern of communication and relating and reports changes/concerns to the health care team and supervisor and/or manager
    • Relates to and shares information with other members of the clients health care team within and outside the organization in a professional and timely manner
    • Reports verbally to the physician/family any changes in the individual’s personal support needs necessitating a change in the care plan
    • Utilizes verbal and non-verbal communication skills effectively with the client
    • Communicates with the clients respectfully and according to their needs and situation/condition
    • Uses effective communication strategies to manage inappropriate, disruptive or aggressive behaviour
    • Consults with the supervisor and/or manager when unable to resolve difficulties with a client or team member or when requiring clarification about an issue
    • Uses interpersonal skills to build caring/helping relationships with clients
    • Acts as a team player, communicating effectively with all team members

     

    Daily Routine:

    • On arrival at work receive report from the departing staff and at the end of the shift give report to the arriving staff
    • Reads all relevant memos, care plans, and other written instructions or directions pertinent to that day’s shift
    • Enters all necessary information in the chart as required
    • Enters relevant information in the staff communication log as required.
    • Medication administration to clients as per CNO standards

  • What You Will Do:

    You will make an impact in the following ways:

    • Assist in the activities of daily living, including eating, dressing, transferring, grooming, and bathing
    • Provide laundering of clothes and linens

    • Are capable of making a commitment of at least 15 hours/week
    • Are flexible to accommodate day shifts from 7:00 a.m. to 3:00 p.m. or evening shifts from 3:00 p.m. to 11:00 p.m. and commit to shifts every second weekend (both Saturday and Sunday). To support planning, we will always make your schedule available two weeks in advance.

    • Assess and treat clients with a wide range of acute and chronic conditions
    • Test and measure the functioning of the musculoskeletal, neurological, pulmonary, and cardiovascular systems.

Job Description
  • What You Will Do:
    • Assess and treat clients with a wide range of acute and chronic conditions
    • Test and measure the functioning of the musculoskeletal, neurological, pulmonary, and cardiovascular systems.
    • Establish a clinical diagnosis and care plan for the treatment of physical problems
    • Document and communicate care plan according to established guidelines and standards

  • What You Will Do:
    • Complete clinically relevant assessments of the client/family and their environment.
    • Develop a care plan with the client and family based on the assessment
    • Document and communicate care plan as well as client care activity per college standards

The Opportunity: Supervisor- Personal Support Services, Temporary

As the Supervisor, Personal Support Services (PSS), you are responsible for daily delivery of personal support services provided by PSWs. You will complete personal support care plans with new clients, update care plans as required, engage with service funders (primarily Ontario Health at Home Care Coordinators), participate in client case conferences, and support the resolution of identified service concerns. Job Description
  • What You Will Do:

    Primary Responsibilities Include

    • Oversees and assists the Manager with the clinical operation of the PSS program
    • Acts as a resource, support and mentor to the PSS staff by leading and coaching staff to attain individual and operational performance objectives
    • Contributes to human resources management by supporting labour and employee relations which includes performance development, management, and recognition.
    • Ensures Carefor policies & procedures affecting the delivery of Personal Support Services are implemented by PSW staff
    • Completes annual performance reviews on assigned Personal Support Services staff
    • Identifies and discusses with the Manager employee issues requiring disciplinary action
    • Assists in determining the required staffing complement based on identified needs
    • Assists Managers in the employee recruitment process by anticipating and monitoring staffing needs and initiating appropriate action to obtain/maintain an adequate compliment of staff
    • Participates in a regular after hours On Call rotation to support urgent operational needs outside of regular business hours.

     Operational And Organizational Responsibilities

    • Understands, promotes, and communicates the value, purpose, and relationship of the Personal Support Services program to Carefor as part of its strategic plan
    • Promotes continuous quality improvement by contributing to the implementation and evaluation of strategies designed to increase the provision of quality care
    • Works in collaboration with client/family to develop the Client Care Plan and complete safety assessments, updates them as required, and communicates changes with the funder and PSS care team.
    • Ensures that the quality of client care delivery is consistent with the funded contractual terms of care & provides regular updates to the funder regarding the care provided
    • Supports PSW scheduling by providing recommendations regarding client care needs
    • Investigates, evaluates and supports the resolution of identified client and staff concerns & complaints

    Health And Safety Responsibilities

    • Operates within and monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures
    • Participates in health and safety training including WHMIS and applies this knowledge
    • Works in collaboration with Management and Human Resources to complete return to work plans for Personal Support Workers taking part in modified work

    Problem Solving

    Problem-solving skills are required to make recommendation to offer solutions to issues and challenges relating to people issues, and Carefor operating policies and practices.

    Decision Making/Independence Of Action

    Makes recommendations for continuous improvement relating to Carefor’s overall objectives, policy, and procedures. Decisions may require review or escalation when costs, potential liability, reputational risk and/or an impact on objectives or Personal Support Services policies are involved.

    Direction is generally received for outliers to organization’s policies, objectives, and values.

    Physical Effort

    • Physical effort required is low intensity, <2 hours per day

    Mental Effort

    • Mental effort required is moderate intensity in an office environment, 2-4 hours per day, or high intensity for less than 2 hours per day.

    Working Conditions

    • Limited exposure to crisis situations
    • Able to travel within the Champlain region (if required)
  • Qualifications
    • A Registered Practical Nurse (RPN), licensed to practice in Ontario, in good standing, is required
    • A minimum 1-year leadership is preferred
    • A minimum of 2 years community experience in Nursing or Personal Support Services
    • Demonstrated leadership abilities with highly developed organizational skills
    • Demonstrated understanding of CNO Standards of Practice
    • Excellent interpersonal and verbal/written communication skills
    • Excellent computer skills/knowledge and familiarity with scheduling systems
    • Ability to work collaboratively with internal and external stakeholders
    • A vehicle and a valid Ontario Driver’s license are required
    • Ability to manage coinciding demands
    • Demonstrated flexibility and adaptability

    Qualifications Considered an Asset

    • Experience working in a health care environment and/or working in a non-profit organization
    • Ability to converse in both official languages (English/French)
    • Experience providing Palliative Care and/or Dementia Care
    • Palliative certification (LEAP)
    • Gentle Persuasive Approach certification (GPA)
    • Previous experience working with unregulated care providers
