Job Description
  • What You Will Do:
    • Responds to the needs of clients and volunteers at the point of contact and for information or referral purposes
    • Effectively communicates and interacts with clients in a courteous and professional manner
    • Accesses and utilizes community resources where appropriate
    • Collaborates with the marketing team to promote events and programs and assists with the development of monthly program calendars
    • Organizes supplies, equipment, and resources for the programs
    • Provides information, coordinates services and/or refers clients/caregivers to other resources as required
    • Initiates and completes the client intake process (including the required assessments and reassessments for specific programs) with guidance from the Centre Leader
    • Maintains and updates client and volunteer files in the client information management system including schedules
    • Assists with volunteer recruitment, selection, orientation, and training, and supports in-service meetings as needed
    • Answers the telephone and greets clients
    • Completes the time sheet data entry for payroll as needed
    • Supports the client survey distribution per the established schedule
    • Processes client payments, donations, and fundraised money
    • Prepares and reconciles bank deposits, and deposits money at the bank as required
    • Participates in fundraising activities which may occur after regular hours
    • Reviews client billable services and volunteer reimbursements before sending to the central finance department
    • Maintains the petty cash
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures.
    • Participates in health and safety training including WHMIS and applies this knowledge in the workplace
    • Wears or uses any/all protective equipment or clothing provided by Carefor and does not interfere with protective devices as to change or negate their functions
    • Reports the absence of or defect in any equipment or protective device, and which may endanger self or other worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
    • Does not use or operate any equipment, machine, device, or work in a manner that may endanger self or other worker
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents (eg. WSIB injuries, infection control)
    • Adheres to the Carefor standards for conduct and behavior and upholds Carefor’s Core Values
    • Ensures that confidential information relating to clients, work colleagues, suppliers and Carefor’s operations is properly protected and not disclosed to third parties, unless allowed or required under relevant law or regulation
    • Treats all people with respect and undertakes duties with care and diligence
    • Does not use information obtained in the course of employment for personal financial gain; nor allows that information be used to obtain financial benefit for any other person or company

  • Qualifications
    • Post-secondary education in Office or Business Administration or similar field, or an acceptable combination of education and experience may be considered
    • 3 years’ experience in a multi-level, not-for-profit organization and/or community support services an asset
    • Experience working with volunteers an asset
    • Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Publisher) and client data base systems (CIMS)
    • Experience in customer service
    • Physical ability to perform duties such as standing for long periods, squatting, bending, and lifting heavy objects
    • Ability to organize, prioritize and multi-task
    • Excellent interpersonal skills and an ability to communicate effectively with clients, caregivers, staff, health care professionals, service agencies both verbally and in writing
    • Bilingualism (English/French) an asset
    • Valid Class G drivers’ license and use of own vehicle

Job Description
  • What You Will Do:

    Clinical Practice

    In all Carefor programs, the RPN provides service consistent with the standards established by Carefor and provincial regulatory agencies/bodies. The nursing process is utilized: assessment, planning, implementation and evaluation.

    • Provides complete, relevant assessment of the client
    • Develops a care plan with the client based on the General and Diabetic Foot Assessment Tool
    • Implements nursing strategies or interventions according to the established care plan
    • Describes, evaluates and documents client’s/family’s responses to care plan utilizing established procedures
    • Utilizes appropriate interviewing and communication skills to obtain information from the client/family
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor documentation standards
    • Evaluates client’s/family’s responses to care plan for the achievement of realistic client-centered outcomes and revises plan as required
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principals and established teaching materials to provide health teaching to clients and families
    • Analyses and incorporates current research findings into clinical practice (e.g.best practices, products & procedures)

  • Qualifications
    • Licensed as a Registered Practical Nurse in the province of Ontario

    • Minimum of nine (9) months to one (1) year of related experience required

Food insecurity and isolation are two major problems facing seniors in our community. With the cost of food and transportation, more and more seniors are finding it harder to age at home. Can you help them?

Every year, the March for Meals campaign helps raise money for Carefor’s meal delivery programs run out of our five senior support centres. Every donation helps provide isolated and vulnerable seniors across your community access to a nutritious meal delivered to their door. But Meal Delivery is more than just ensuring nutrition; it’s also about making sure someone is doing okay and is a little less alone.

Your donation helps make this possible helping Carefor with the cost of operating our meal delivery program. You’ll be helping put a meal on a table and smile on a face.

Carefor is launching the Caregiver Speaker Series—an educational and inspirational initiative designed to support those caring for loved ones. Funded by Rexall, this six-part series will be hosted across Carefor’s centres in Eastern Ontario, at no cost to attendees.

Each session will feature Carefor’s frontline experts—occupational therapists, nurses, personal support workers, and social workers—sharing practical strategies for dementia care, safe mobility, and navigating community resources. The series offers caregivers not only knowledge, but affirmation and connection.

Topics will be decided based on consultation with each Centre team. They are experts in their communities and will know or may suggest topics that will create the most interest.

With 71 percent of Canadian caregivers reporting increased burnout since the pandemic, this program provides a vital space for learning, rest, and renewal. While caregivers attend, Carefor will provide on-site support for their loved ones, ensuring peace of mind and uninterrupted care.

  • What You Will Do:

    Responsibilities:

    • Follow the individual’s care plan and inform a supervisor if care needs have changed.
    • Contributes to the development of the client’s Care Plan, which is created by the Personal Support Program Supervisor.
    • Performs personal support tasks reflecting the needs of the individual/family. Tasks may include:
    • light housekeeping duties,
    • meal set-up or planning and preparing nutritious meals including special diets,
    • laundering of clothes and linens,
    • monitoring and maintaining household safety,
    • providing personal care which could include bathing, mouth and skin care, dressing and grooming, assistance in walking and transfers, climbing or descending stairs, feeding, exercises and bedside care,
    • perform delegated tasks (e.g. applying prescription ointment) where required, under the direction of the supervisor or another Professional (RN, PT, OT),
    • recognizing the social and emotional needs of clients and dealing with them effectively,
    • providing respite care, accompanying clients on outings or to appointments as necessary, shopping for client as required,
    • Planning and organizing each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment, completing assignments in the assigned length of time,
    • Receives direction and assignments from the coordinator or supervisor by phone, voicemail, email or in person.
    • Consults with the coordinator or supervisor regarding assignment changes, referral changes/differences and other pertinent information.
    • Reports verbally to the supervisor any changes in the individual’s personal support needs or home situation requiring a change in the care plan.
    • Completes daily call a-heads and reports any changes to schedule to the coordination team immediately.
    • Observes the client’s pattern of communication and relating and reports changes/concerns to the supervisor.
    • Utilizes verbal and non-verbal communication skills effectively with the client.
    • Consults with the supervisor when unable to resolve difficulties with a client or team member or when requiring clarification about an issue.
    • Listen to voice mail message and check emails regularly and responds promptly when required.
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures.
    • Participates in health and safety training including WHIMS and applies this knowledge in the workplace.

  • What You Will Do:

    The Personal Support Worker is a critical role within our communities as it positively affects the lives of clients and ignites the warmth of human connection. You will be making a difference in the lives of your clients each day by providing personal care, delegated exercise programs, and activities of daily living (ADLs) as prescribed by each client’s individual care plan while collaborating with all members of the health care team to meet the needs of individuals to attain their optimal level of independence and well-being.

She holds post graduate certificates in the areas of Elder Planning Counselor®️, Certified Health Insurance Specialist™️, and Master Financial Adviser – Philanthropy (MFA-P™️) and is also in the process of completing her Certified Financial Planner (CFP®️) designation. 


Erin has been featured on CTV Ottawa News for her volunteer work with the Junior Diabetes Research Foundation and is the proud past recipient of the Mayor’s Award for Youth Volunteerism from the City of Kingston. In 2023, Erin was also awarded the Women in Capital Markets Emerging Leaders Award and currently serves on the board of the Ottawa Chapter of the Canadian Association of Gift Planners. 

Erin is the proud mom of two amazing boys, Jude and Jakob, and three Airedale/Giant Schnauzer pups, who keep her on her toes. She lives just outside of the city of Ottawa in the beautiful small town of Merrickville but, you won’t often find her there as she and her husband, Ted, are happily either rink side or diamond side most evenings and weekends!

It’s time to March for Meals !

Food insecurity and isolation are two major problems facing seniors in our community. With the cost of food and transportation, more and more seniors are finding it harder to age at home. Can you help them?

Your donation will help provide isolated and vulnerable seniors across your community access to a nutritious meal delivered to their door. But Meal Delivery is more than just ensuring nutrition; it’s also about making sure someone is doing okay.

Your donation helps change this. You’ll be helping put a meal on a table and smile on a face.

Our seniors have looked after us their whole lives. It’s time for us to look after them.

March for Meals: Helping Corrie Stay Healthy and Connected

The Meal Delivery Program at the Lan‑Char Support Centre in Lancaster begins each day with something simple but powerful: neighbours looking out for one another. With up to one-third of seniors living alone, a warm meal delivered with a friendly smile can mean the difference between isolation and connection. For many seniors, these meals are not only nourishment, but are also a reminder that someone cares.

Nancy Contant has seen this impact firsthand. After more than 25 years volunteering with Carefor, first through footcare and now through meal delivery, she knows how meaningful a brief visit can be. Living in Lancaster and caring full‑time for her 100‑year‑old mother, Nancy says the program enriches her life as much as it does the clients’. “Meeting all the clients and making a difference in their lives” is what keeps her going, she explains. It’s also how she met Marilyn, who has become one of her closest friends. Volunteering, she adds, gives her a chance to recharge and is “a social outing to help others” that doubles as caregiver support for herself.

Marilyn Lebrun joined the program two years ago, shortly after retiring and welcoming a new granddaughter. Encouraged by Nancy, she quickly discovered how fulfilling the work could be. “A lot of people know me in the community, so it’s a great social outing,” she says. “It feels good to give back.” Like Nancy, she treasures the friendships she’s formed along the way.

One of the people they visit regularly is Corrie Flipsen. Corrie lives alone, though she has a large and loving extended family. The Meal Delivery Program allows her to maintain her independence without relying on them for daily help. She loves the hot meals, but even more, she loves the visits. “Meeting new people and socializing with the volunteers gives me something to look forward to,” she says.

These stories reflect the heart of Carefor’s five Senior Support Centres in Alexandria, Lancaster, Ingleside, Winchester, and Finch. Each centre provides essential services that help seniors age safely at home, stay socially connected, and feel supported in their community. As Lan‑Char Support Centre supervisor Tina Castonguay puts it:
“Our meal delivery program means that many seniors can stay at home, where they want to and for some, where they need to due to the cost of retirement homes. We’re proud to help our seniors live with independence.”

Hike for Hospice for Carefor Hospice Cornwall is back for another year! This annual walk is a major event in our calendar to help us raise essential funds for our hospice so we can continue to provide end-of-life care for our clients and their families.

Your participation and support mean everything, so we hope you’ll either lace up your walking shoes or support someone who is.

This year’s Hike will take place on Sunday, May 5th at Carefor Hospice Cornwall at 1507 Second St W in Cornwall.  Registration opens at 12:30 PM, and the Hike starts at 1:00PM, with a BBQ to follow.
