The Opportunity: Supportive Recreation Worker – Grief & Loss Part Time, Permanent

As a member of the Palliative Community Programs Team, the Supportive Recreation Worker – Grief & Loss, supports individuals through their grief journey and works in conjunction with the Patient and Family Support Services Counsellor. This role functions as a member of a multidisciplinary team and is responsible for planning, coordinating, and delivering recreational and therapeutic activities supporting grief, bereavement, and emotional well-being for Hospice clients, families, and staff. This position also supports social casework, case management, and group facilitation as appropriate. Care is provided within the framework of quality and risk management and in accordance with Carefor’s policies, Hospice Palliative Care Ontario (HPCO) standards, and the professional guidelines set by the Ontario College of Social Workers and Social Service Workers.

** This is a part-time position, scheduled for 37.5 hours per pay period, requiring evening and weekend availability as needed.

Job Description
  • What You Will Do:
    • Plan, implement, and evaluate a variety of therapeutic recreation and legacy building activities.
    • Provide non-clinical, emotional support to patients and caregivers experiencing grief and loss.
    • Facilitate group sessions
    • Educate families and caregivers regarding the grief process, coping strategies and available resources
    • Communicate effectively with patients, caregivers, staff, and the community to encourage participation in programs.
    • Support and coordinate volunteer involvement.
    • Assess client’s needs to determine eligibility for services and development of care plans that reflect the needs and preferences of the individual; detail appropriate support and interventions; and ensures a wellness approach to care.
    • Refer client to partner organizations and programs as applicable.
    • Utilize a care coordination approach to monitor and respond to changes in needs of the client including but not limited to updated care plans, changes in status, and internal/external referrals.
    • Maintain contact with assigned volunteers, monthly or more as required, to document volunteer interactions, provide support, and communicate updates or changes in client status and/or care needs.
    • Support grief and bereavement training with Palliative Care Education and training tools recognized by HPCO.
    • Represent the organization at outreach functions
    • Abides by the practice standards of Social Work and Carefor policies and procedures
    • Informs clients of foreseeable risks as well as rights, opportunities and obligations associated with the provision of Social Work services
    • Facilitate links with other program teams, incorporate, and strengthen collaborative and multi-disciplinary teamwork
    • Assist in the implementation of regularly scheduled Quality Assurance initiatives such as Chart Audits, Peer Reviews, and Best Practice Guidelines
  • Qualifications
    • Post-secondary diploma/degree in Recreation & Leisure Studies, Therapeutic Recreation, Gerontology, Social Work or Social Service Work
    • Registration with or eligibility for registration with Therapeutic Recreation Ontario (TRO) and/or registered with Ontario College of Social Workers and Social Service Workers if applicable
    • Strong leadership, organizational, and interpersonal skills
    • 2-3 years of experience (specifically relevant experience in hospice palliative care, social services, and/or community support sector)
    • Experience in developing, implementing, delivering, and evaluating individual, group, and community programs
    • Professional experience in program coordination, client relations, client intake process, client assessments, care planning, and data collection
    • Experience in working within an interdisciplinary team that includes volunteers
    • Demonstrated ability to value diversity, be client-centered, express flexibility, and maintain a positive attitude
    • Experience in providing individual and group counseling and interventions
    • Proficiency and experience working in a computerized environment
    • Bilingual (French and English) strong asset
    • Experience with grief and bereavement, end of life
    • Experience in hospice care, knowledge of hospice/palliative care philosophy
    • Collaborate with all staff to ensure quality development, and delivery of hospice program
    • Establish and maintain community relationships for referrals and program development
    • Experience with Client Intake and Case Management
    • Experience with Group Facilitation and training for grief support
    • Participation in fund development and public engagement events
    • Valid Driver’s Licence and reliable transportation

    The hiring range for this position is $27.57- $31.59 /per hour which reflects the expected starting salary based on qualifications and experience. The Opportunity: Registered Practical Nurse- Permanent

Reporting to the Manager of Allied Health Services, the Registered Practical Nurse, atHome Programs is responsible for the provision and management of quality nursing care to clients transitioning out of hospital as part of the Carefor’s atHome bundled care programs. The Registered Practical Nurse atHome programs is responsible for managing the wrap-around care that is coordinated and delivered to clients admitted to the program, with support from members of the care team. The Registered Practical Nurse will be completing InterRAI-HC assessments, participating in the development of care plans, engaging with other members of the care team (e.g., social workers, PTs, OTs, PTA/OTA, nutritional specialists, and speech language pathologists) and delegating tasks to personal support workers assigned to deliver care. Of note, the Registered Practical Nurse will utilize InterRAI-HC assessment findings to proactively develop care plans supporting interventions aimed at minimizing unnecessary emergency department visits by maintaining clients in their home with the necessary support.

The Registered Practical Nurse is also responsible for providing clinical support, identifying and following up with PSW job performance issues, participating in client case conferences, offering ongoing training and assessments, and providing feedback on performance.
As a resource to the atHome Program, the Registered Practical Nurse will support the care team and clients in providing safe, client-centered care by modeling practice standards, defining care processes and procedures, and collaborating in the design of personal support workers’ education and training.

  • What You Will Do:
    • Supports the care management and home care assessment responsibilities of clients transitioning out of hospital within the atHome Program;
    • Delivers nursing care to clients within the atHome Program;
    • Acts as a resource to the atHome Program care team, providing coaching, education, and instruction to encourage individual and operational performance objectives. Encourages and models evidence-informed practice;
    • Supports the performance management process with assigned staff and engages with appropriate managers regarding any performance issues;
    • Contributes to human resources management including employee relations, health and safety, continuing education, performance development, recognition of staff and orientation, as required;
    • Assists the Manager in identifying areas for recruitment, anticipates and monitors staffing needs and initiates appropriate action to obtain/maintain an adequate compliment staff for the atHome program;
    • Investigates and evaluates client and staff concerns/complaints and implements appropriate resolutions, as required

    Operational And Organizational Responsibilities

    • Leads by example and promotes continuous quality improvement;
    • Understands, promotes, and communicates the value, purpose, and goals of the atHome Program;
    • Working with the Allied Health Services Manager in the evaluation and revision of atHome Program policies and procedures, based on changes in practice and requirements from funder or partner organization;
    • Ensures that the quality of client care delivery is consistent with Carefor’s mission, vision, values and strategic plan;
    • Meets the contractual terms set forth by external/funding partners and maintains communication with these partners as needed;
    • Assists with operation of the program; provision of direct client care as required;
    • Collaborate to address quality, safety and risk management issues, including the creation of professional development and performance improvement plans to address shortfalls;
    • Contributes to the improvement of client outcomes (quality of life and quality of care) based on best available evidence and efficient utilization of resources;
    • Contributes to business development initiatives, as required;
    • Support the atHome Program team through regular meetings, orientation, mentorship, consultation, and guidance with complex clinical situations. Participate in internal and external case conferences or meetings as required; and
    • Operates within as well as monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures.

    Working Conditions

    • Combined regular office environment with some flexibility of remote work.
    • A vehicle for travel within the Ottawa region to support Nursing staff working in various community settings (e.g. individual homes, retirement homes, Carefor clinics) will be required.

  • Qualifications
    • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario; required.
    • Three (3) years recent experience of working within the adult home health care sector with related experience to support the provision of restorative care to older adults.
    • Knowledge of the related regulatory and legislative requirements within Ontario; required.
    • Certification in clinical specialties and chronic disease management (e.g., GPNC(C) Gerontology Practical Nurse Certified (C) Canada or CMSN(C) Medical-Surgical Nursing (C) Canada); an asset.
    • Ability to promote and facilitate evidence-based practice within a community setting with a demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.
    • Superior communication skills and ability to communicate effectively to direct and motivate others while maintaining a persuasive and credible presentation style at all levels of the organization.

    • Excellent organizational, planning and time management skills.

  • What You Will Do:

    You will make an impact in the following ways:

  • What You Will Do:

    Responsibilities:

    • Follow the individual’s care plan and inform a supervisor if care needs have changed.
    • Contributes to the development of the client’s Care Plan, which is created by the Personal Support Program Supervisor.
    • Performs personal support tasks reflecting the needs of the individual/family. Tasks may include:
    • light housekeeping duties,
    • meal set-up or planning and preparing nutritious meals including special diets,
    • laundering of clothes and linens,
    • monitoring and maintaining household safety,
    • providing personal care which could include bathing, mouth and skin care, dressing and grooming, assistance in walking and transfers, climbing or descending stairs, feeding, exercises and bedside care,
    • perform delegated tasks (e.g. applying prescription ointment) where required, under the direction of the supervisor or another Professional (RN, PT, OT),
    • recognizing the social and emotional needs of clients and dealing with them effectively,
    • providing respite care, accompanying clients on outings or to appointments as necessary, shopping for client as required,
    • Planning and organizing each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment, completing assignments in the assigned length of time,
    • Receives direction and assignments from the coordinator or supervisor by phone, voicemail, email or in person.
    • Consults with the coordinator or supervisor regarding assignment changes, referral changes/differences and other pertinent information.
    • Reports verbally to the supervisor any changes in the individual’s personal support needs or home situation requiring a change in the care plan.
    • Completes daily call a-heads and reports any changes to schedule to the coordination team immediately.
    • Observes the client’s pattern of communication and relating and reports changes/concerns to the supervisor.
    • Utilizes verbal and non-verbal communication skills effectively with the client.
    • Consults with the supervisor when unable to resolve difficulties with a client or team member or when requiring clarification about an issue.
    • Listen to voice mail message and check emails regularly and responds promptly when required.
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures.
    • Participates in health and safety training including WHIMS and applies this knowledge in the workplace.
    • Participates in all Mandatory Education
    • Wears and uses any/all protective equipment or clothing provided by Carefor and does not interfere with protective devices to change or negate their functions.
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware, and which may endanger him/herself, another worker or the client.
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard to their supervisor or coordinator.
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents.
