  • What You Will Do:

    As a Registered Practical Nurse in Carefor’s Visit Nursing Program, you will make a difference in the lives of your clients in the community by utilizing your excellent communication skills to obtain relevant information from your clients and their families. You will implement nursing strategies or interventions according to the established care plan in consultation with the client and/or caregiver and the health care team. You will utilize adult learning principals and teaching materials to provide guidance to clients and their caregivers. You will incorporate your research findings into your daily clinical practice while demonstrating your exceptional time management and advance planning skills that will promote client-directed care, self-care and independence.

  • What You Will Do:
    • Assists or provides care according to the Care Plan and in compliance with a CCW’s scope of practice. And reflects the needs of the client.
    • Provides personal care which includes assistance with all aspects of activities of daily living including but not limited to: bathing, dressing and grooming, mouth and skin care, lifts and transfers, light housekeeping including laundry and tidying, assisting with medications or other delegated tasks, exercises, assists with climbing or descending stairs, walking, etc.
    • Escorts residents to and from dining room, assists with feeding and serving the residents.
    • Observes client for changes in behaviour, signs or symptoms of decreasing health status and gives verbal/written report to RPN or other team members.
    • Evaluates the individual’s need for change in their personal support care plan and makes recommendations to RPN or designate.
    • Recognizes the social and emotional need of the clients and deals with them effectively.
    • Contributes to the development of the personal support care plan by reporting, to the RPN or designate, all observations or information obtained from the resident and/or family regarding needs and the effect of the care he/she provided.
    • Plans and organizes each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment.
    • Assists with Meal set-up, planning and preparing nutritious meals including special diets.
    • Performs light housekeeping/cleaning duties. Including laundry.
    • Makes appointments for clients and accompanies them on outings when necessary.
    • Encourages and promotes client independence and treats clients with dignity and respect.
    • Completes documentation according to established procedures to maintain the communication to other health team members and provide an accurate account of service provided
    • Reviews all forms of documentation and communication in order to remain current on residence operations and resident care. This may include reviewing care notes, e-mails, memos, communication book, and receiving verbal updates.
    • Informs the manager or designate if they require additional training with tasks.

  • What You Will Do:
    • Maintaining a clean, safe environment
    • Performing tasks according to duty sheet
    • Setting up equipment and supplies for food preparation

  • What You Will Do:

    The Personal Support Worker is a critical role within our communities as it positively affects the lives of clients and ignites the warmth of human connection. You will be making a difference in the lives of your clients each day by providing personal care, delegated exercise programs, and activities of daily living (ADLs) as prescribed by each client’s individual care plan while collaborating with all members of the health care team to meet the needs of individuals to attain their optimal level of independence and well-being.

    You will make an impact on the lives of clients in the following ways:

    • Assist in the activities of daily living including eating, dressing, transferring, toileting, grooming, and bathing
    • Provide cognitive, social, and compassionate support
    • Document reports for clients on their plan of care and any changes in the behavior of daily routines

    To provide optimal service to our clients as a Part-Time Personal Support Worker, we require that you are capable of making a commitment to accommodate 37.5 hours bi weekly. Must be able to work on shift rotations days and evening and weekends, every second weekend.

    A standard schedule will be respected, subject to changes as required by program demand, changes in client visits and caseload. To support planning, we offer you a schedule 2 weeks in advance.

  • Qualifications
    • Certificate in Home Support Worker (Level II, III), Personal Support Worker or Developmental Service Worker program, Health Care Aide, Foreign-Trained BScN or Doctor, Paramedic Graduate, BScN Nursing Student, or second year RPN student.
    • National Academy of Careers Colleges Certificate, if applicable
    • Your ability to manage confidential information
    • Your ability to work independently
    • Your ability to communicate effectively in English, both written and verbal
