The Opportunity: Physiotherapist Assistant- Full-time

The Physiotherapy Assistant (PTA) provides clinical care for clients under the direction and supervision of the Physiotherapist. The PTA works to actively rehabilitate each client to achieve his/her maximum functional level through planned and supervised therapeutic programs. The PTA is responsible for monitoring the progress of clients and reporting to the physiotherapist. The PTA may perform associated functions to support service delivery such as stocking supplies or maintaining equipment. The PTA ensures the safety, comfort and privacy of clients. As a member of a team, the PTA participates in discipline-specific and interdisciplinary activities that are directed towards a client-centered care environment and that contributes to operational efficiency, professional and program development.

The PTA may work autonomously in a variety of settings including the community delivering services to clients and families by following the treatment plan of the Physiotherapist. Thorough knowledge of the scope of practice of a PTA and good judgment is critical in this role.

Job Description
  • What You Will Do:
    • Learn programs of care from the Physiotherapist and provide care to individual clients based on the frequency specified in the program of care
    • Lead a group of clients in a specified group exercise program at the frequency and time specified by the Physiotherapist
    • Monitor the client’s ability and accommodate any changes from baseline. Report back to the Physiotherapist the need to change/adjust the program, therapeutic modalities
    • Understand and follow all directions and specifics related to the application of modalities
    • Ensure client safety at all times
    • Complete written reports as required
  • Qualifications
    • Year Two Physiotherapy Assistant Diploma from an institution recognized by the College of Physiotherapists of Ontario or an undergraduate degree in human kinetics.
    • Minimum of 2 years recent related experience in adult physical medicine gained in a community, rehabilitation or long-term care setting
    • Ability to speak, read & write the English language
    • Proficiency in using a computer for email and word documents

The Opportunity: Physiotherapist – Full-time

    • Assess and treat clients with a wide range of acute and chronic conditions
    • Restore movement and function through exercise, strength training, mobility retraining, and balance programs
    • Support rehab needs, including musculoskeletal, neurological, pulmonary, and cardiovascular care

Job Description
  • What You Will Do:
    • Assist or provide care according to the Resident Care Plan. Assistance may need to be provided on a short or long term basis to individual residents including but not limited to personal care, lifts and transfers, observing nutritional status, light housekeeping, assist in taking prescribed medication, etc…
    • Observe resident for changes in behaviour or signs or symptoms of decreasing health status and report to Care Team leader.
    • Chart care given and give verbal/written report to RPN or other team members on any unusual behaviour and observations made with respect to clients/residents
    • Evaluate the individual’s need for change in the personal support care plan and make recommendations to RPN or designate.
    • Contributes to the development of the personal support care plan by reporting, to the RPN or designate, all observations or information obtained from the resident and/or family regarding needs and the effect of the care he/she provided.
    • Plans and organizes each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment.
    • Completes documentation/paperwork according to Carefor policies and procedures
    • Reviews all forms of documentation and communication in order to remain current on residence operations and resident care. This may include reviewing care notes, e-mails, memos, communication book, and receiving verbal updates

Job Description
  • What You Will Do:

    Client-centred support and assistance with activities of daily living (ADLs), personal care and other care needs assigned. Duties will be carried out under the formal delegation of a registered nursing professional, in accordance with organizational policies, applicable legislation, and within the scope of training and competence. Responsibilities may include, but are not limited to the following:

    • Personal Care & ADLs: Assist clients with bathing, grooming, dressing, toileting, mobility, and safe transfers (including use of lifts and mobility aids).
    • Health Monitoring: Observe and report changes in client condition to the supervising nurse or care team.
    • Medication Assistance: Provide medication reminders or assistance as per care plan.
    • Environmental Support: Perform light housekeeping, laundry, and meal preparation to maintain a safe and clean environment.
    • Social & Emotional Support: Engage clients in conversation and activities to reduce isolation and promote well-being.
    • Documentation & Communication: Accurately document provided care and communicate effectively with clients, families, and team members.
    • Safety & Infection Control: Follow Personal Protective Equipment (PPE) protocols, infection prevention practices, and health and safety regulations.

    • Provides complete, relevant assessment of the resident and describes and/or documents finding according to documentation procedures.
    • In collaboration with the Manager or designate, prepares and updates the resident profile and care plan

  • What You Will Do:
    • Provides complete, relevant assessment of the client’s functional abilities, general condition and activities of daily living following pre-set procedures
    • Implements the care plan based on the nursing assessment in collaboration with the physician, PSW’s and manager and review quarterly
    • Implements nursing strategies or interventions according to the established care plan
    • Performs technical nursing skills at a safe level in accordance with scope of practice, nursing principles and Carefor policies and procedures
    • Utilizes appropriate interviewing and communication skills to obtain information from the client/family
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor standards (by exception)
    • Evaluates client’s responses to care plan for the achievement of realistic client-centered outcomes; revises plan as required
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principles and established teaching materials to provide health teaching to clients, unregulated care providers and volunteers
    • Reports and discusses client’s status on a regular basis with other members of the health care team, including physician when necessary
    • Works closely with the care team (including psws, cooks, recreation/activity coordinator and/or volunteers), providing them with direction, teaching and support
    • Orders and checks new medications from the pharmacy and lists the medications on the MAR sheets
    • Admissions and Discharges of clients as required
    • Keep up to date on changes in the field of dementia care
    • Familiarity with the home’s fire, disaster and evacuation plans
    • Participate in providing personal care, activities of daily living, preparing and serving meals & snacks, light housekeeping, recreational/personal interest activities as necessary.

    • Three years experience working with elderly clients with dementia

Job Description
  • What You Will Do:
    • Provides complete, relevant assessment of the client utilizing pre-set procedures
    • Develops a care plan with the client and family based on the nursing assessment, incorporating discharge planning
    • Implements nursing strategies or interventions according to the established care plan
    • Performs technical nursing skills at a safe level in accordance with nursing principles and Carefor’s policies and procedures
    • Describes, evaluates and documents client/family responses to care plan utilizing established procedures and
    • the achievement of realistic client-centered outcomes, and revises the plan as required
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor documentation standards
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principals and established teaching materials to provide health teaching to clients and families
    • Reports and discusses client’s status on a regular basis with other members of the health care team
    • Acting as a wound champion to manage wounds based on best practices
  • Qualifications
    • Registration with the College of Nurses in Ontario
    • High tech skills required
    • Experience with IV restart
    • Palliative experience preferred
    • LEAP training or willing to take as a requirement to palliative care provision
    • Experience in community health care preferred
    • Excellent documentation skills
    • Ability to problem solve and to adapt nursing to independent practice
    • Ability to work collaboratively as a member of an interdisciplinary practice
    • Possession of a valid driver’s license, (G2 level)
    • Use of a reliable vehicle that is insured for business use required
    • Bilingualism (French/English) an asset

Job Description
  • What You Will Do:

    As a Registered Practical Nurse in Carefor’s Visit Nursing Program, you will make a difference in the lives of your clients in the community by utilizing your excellent communication skills to obtain relevant information from your clients and their families. You will implement nursing strategies or interventions according to the established care plan in consultation with the client and/or caregiver and the health care team. You will utilize adult learning principals and teaching materials to provide guidance to clients and their caregivers.   • Qualifications
    • You are registered in good standing with the College of Nurses in Ontario (CNO)
    • You have excellent high tech skills (with IV restart) or a passion to be trained in these skills
    • You possess a valid driver’s license and the use of a reliable vehicle
    • You have excellent documentation skills
    • You have the ability to work collaboratively as a member of an interdisciplinary practice

    • Ability to converse in both official languages (English/French)

Job Description
  • What You Will Do:
    • Provides complete, relevant assessment of the resident and describes and/or documents finding according to documentation procedures.
    • In collaboration with the Manager or designate, prepares and updates the resident profile and care plan
    • Effectively and safely administers medication according to Carefor procedures and CNO standards of practice
    • Collaborates with other members of the health care team to ensure residents receive quality care
    • Identifies problems using a specified framework and in consultation with the Manager or designate and resident, selects and implements individualized strategies or interventions in order to focus care on resolving or managing the problem
    • Describes, evaluates and documents resident’s responses to care
    • Describes rationale for referrals and changes in Service Plan
    • Provides cost-effective, organized, outcome directed care on an ongoing basis
    • Identifies and utilizes tools and resources for work and manages these resources through regular monitoring of equipment etc. and reporting unusable equipment to supervisor or designate.
    • Delegates care appropriately
    • Meets with resident and family to orient them to the residence.
    • Seeks input from residents to assure that service meets expectations
    • Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
    • Functions in accordance with the standards of nursing practice from the College of Nurses of Ontario
    • Functions in accordance with the Carefor policies and procedures and the strategic direction of Carefor Pembroke-Renfrew County
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents
  • Qualifications
    • RPN Diploma from approved college
    • Valid registration with the College of Nurses of Ontario
    • Certificate in gerontology would be an asset.
    • Current CPR and First Aid
