The Client Services Coordinator works as part of a team to implement Carefor’s approved or prescribed processes to ensure every client visit is properly scheduled, every service provider’s caseload is properly assigned, and client expectations are met. The Client Services Coordinator also maintains accurate documentation and communications with clients, service providers, and others users. A Client Services Coordinator may have to manage client expectations to ensure the expected service level is achieved.

Job Description
  • What You Will Do:
    • Schedules service providers for client visits in areas assigned by the manager.
    • Provides work schedules to staff that meet client needs and maximize continuity of care.
    • Keeps up to date with daily changes such as staff availability, changes in client service
    • authorizations and/or care plans
    • Ensures client and service provider information is accurate and up to date in the
    • scheduling database
    • Provides timely information, through the use of communication technology (telephone,
    • fax, e-mail, voicemail), to field staff, service providers, co-workers, Managers and
    • Supervisors, clients, and customers
    • Verifies visit confirmations from field staff and processes in a timely manner to be
    • processed by the billing department
    • Verifies payroll information for field staff in a timely manner ensuring the accuracy of hours,
    • mileage, stats, and premiums.
    • Contacts LHIN regarding discrepancies in frequency, authorization for extra visits or not
    • seen/ not found visits, extensions to planned review dates
    • Works collaboratively as part of a team willing to assist others and mentor new staff
    • Contributes to the principles of customer-focused service and continuous quality
    • improvement
    • Functions and completes all duties in compliance with Occupational Health and Safety
    • legislation, regulations, and Carefor policies, procedures, and standards
  • Qualifications
    • Diploma or Degree from an accredited College or University
    • Previous scheduling experience preferred
    • Experience using database software and Microsoft Office Suite
    • Accurate and efficient computer data-entry skills
    • Knowledge of medical terminology is considered an asset
    • Demonstrated problem-solving skills
    • Excellent interpersonal skills

Job Description
  • What You Will Do:

    Client-centred support and assistance with activities of daily living (ADLs), personal care and other care needs assigned. Duties will be carried out under the formal delegation of a registered nursing professional, in accordance with organizational policies, applicable legislation, and within the scope of training and competence. Responsibilities may include, but are not limited to the following:

    • Personal Care & ADLs: Assist clients with bathing, grooming, dressing, toileting, mobility, and safe transfers (including use of lifts and mobility aids).
    • Health Monitoring: Observe and report changes in client condition to the supervising nurse or care team.
    • Medication Assistance: Provide medication reminders or assistance as per care plan.
    • Environmental Support: Perform light housekeeping, laundry, and meal preparation to maintain a safe and clean environment.
    • Social & Emotional Support: Engage clients in conversation and activities to reduce isolation and promote well-being.
    • Documentation & Communication: Accurately document provided care and communicate effectively with clients, families, and team members.

Job Description
  • What You Will Do:
    • Orders required food items and supplies per the planned menu
    • Collaborates on the development of menus for various themed activities
    • Ensures all food orders are received, covered, labelled, dated, and stored properly
    • Follows standardized recipes for the planned menu including special diets
    • Thaws meat and other frozen foods as required
    • Cleans all produce needed for food preparation
    • Prepares meats, desserts, pastries, soups, sauces, vegetables, salads, cereals, eggs, and luncheon items according to the menu and hours of service
    • Cooks food according to required temperature/methods
    • Stores all leftover foods at the required temperature and in appropriate containers (noting dates for rotation purposes)
    • Adheres to standard portions determined for the program

  • Qualifications
    • Minimum Grade 12 or equivalent.
    • Food Handlers Certificate
    • 3 years’ experience in food preparation in a commercial or retirement residential setting
    • Experience in entrée, soup, baking and salad production from fresh and/or frozen
    • food/produce/meats.
    • Experience with preparing food for regular and special diets
    • Experience with use of various kitchen equipment
    • Working knowledge of the Canada Food Guide
    • Ability to understand oral and written instructions and to calculate recipe quantities
    • Able to lift 40 pounds, to stand for long periods of time and walk short distances
    • Ability to tolerate heat in kitchen area

    The hiring range for this position is $40,404.00 to $42,276.00 per year, which reflects the expected starting salary based on qualifications and experience. The Opportunity: Rapid Response Nurse- Full-Time Permanent

Posting Date: November, 2025

Reporting to the Manager of Allied Health Services, the Registered Practical Nurse, atHome Programs is responsible for the provision and management of quality nursing care to clients transitioning out of hospital as part of the Carefor’s atHome bundled care programs. The Registered Practical Nurse atHome programs is responsible for managing the wrap-around care that is coordinated and delivered to clients admitted to the program, with support from members of the care team. The Registered Practical Nurse will be completing InterRAI-HC assessments, participating in the development of care plans, engaging with other members of the care team (e.g., social workers, PTs, OTs, PTA/OTA, nutritional specialists, and speech language pathologists) and delegating tasks to personal support workers assigned to deliver care. Of note, the Registered Practical Nurse will utilize InterRAI-HC assessment findings to proactively develop care plans supporting interventions aimed at minimizing unnecessary emergency department visits by maintaining clients in their home with the necessary support.

The Registered Practical Nurse is also responsible for providing clinical support, identifying and following up with PSW job performance issues, participating in client case conferences, offering ongoing training and assessments, and providing feedback on performance.
As a resource to the atHome Program, the Registered Practical Nurse will support the care team and clients in providing safe, client-centered care by modeling practice standards, defining care processes and procedures, and collaborating in the design of personal support workers’ education and training.

    • Supports the care management and home care assessment responsibilities of clients transitioning out of hospital within the atHome Program;
    • Delivers nursing care to clients within the atHome Program;

    • Contributes to the improvement of client outcomes (quality of life and quality of care) based on best available evidence and efficient utilization of resources;

    Working Conditions:

    • Combined regular office environment with some flexibility of remote work.
    • A vehicle for travel within the Ottawa region to support Nursing staff working in various community settings (e.g. individual homes, retirement homes, Carefor clinics) will be required.

  • Qualifications
    • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario is required.
    • Three (3) years recent experience of working within the adult home health care sector with related experience to support the provision of restorative care to older adults.
    • Knowledge of the related regulatory and legislative requirements within Ontario is required.
    • Certification in clinical specialties and chronic disease management (e.g., GPNC(C) Gerontology Practical Nurse Certified (C) Canada or CMSN(C) Medical-Surgical Nursing (C) Canada) is an asset.
    • Ability to promote and facilitate evidence-based practice within a community setting with a demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.
    • Superior communication skills and ability to communicate effectively to direct and motivate others while maintaining a persuasive and credible presentation style at all levels of the organization.
    • Demonstrated effective analytical, and problem-resolution skills.
    • Excellent organizational, planning and time management skills.

    • Oversees and assists the Manager with clinical operation of the ADP and CSS programs
    • Acts as a resource, support and mentor to the ADP and CSS staff by leading and coaching staff to attain individual and operational performance objectives

    • Follow the individual’s care plan and inform a supervisor if care needs have changed.
    • Contributes to the development of the client’s Care Plan, which is created by the Personal Support Program Supervisor.
