Job Description
  • What You Will Do:
    • Maintaining a clean, safe environment
    • Performing tasks according to duty sheet
    • Setting up equipment and supplies for food preparation
    • Assists in prepares meals, dessert, and/or snacks following standardized approved recipes, being careful to minimize waste and leftovers, and cook food according to sanitation and standardized guidelines pertaining to food preparation
    • Cleanup of kitchen and dining area
    • Serving meals to clients/residents
    • Ensuring timely service of meals
    • Handling and storing all left over foods according to food handling guidelines
    • Makes observations of resident behaviour and intervenes according to job expectation e.g. nutritional intake, disruptive behaviour in dining room, attendance for meals, general concerns affecting operations of the dietary department
    • Report observed concerns to supervisor or designate
    • Becoming familiar with individual resident’s dietary preferences, allergies, and diet restrictions
    • Assisting residents, if needed, to cut food or pour beverages
    • Promptly reports any problems, accidents, emergency situations, and complaints of residents regarding service to the supervisor or designate
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents
    • Performing other duties/tasks as assigned by manager
  • Qualifications
    • Completion of Grade 10 or equivalent is preferred
    • Food Handling and Safety certificate
    • WHMIS certification

The Registered Practical Nurse (RPN)- Wellness & Foot Care is responsible for the provision of wellness support and management of cost-effective foot care services in Community Centers located in Lancaster, Alexandria, Finch, Ingleside and Winchester or the client’s home. Job Description
  • What You Will Do:

    Clinical Practice

    In all Carefor programs, the RPN provides service consistent with the standards established by Carefor and provincial regulatory agencies/bodies. The nursing process is utilized: assessment, planning, implementation and evaluation.

    • Provides complete, relevant assessment of the client
    • Develops a care plan with the client based on the General and Diabetic Foot Assessment Tool
    • Implements nursing strategies or interventions according to the established care plan
    • Describes, evaluates and documents client’s/family’s responses to care plan utilizing established procedures
    • Utilizes appropriate interviewing and communication skills to obtain information from the client/family
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor documentation standards
    • Evaluates client’s/family’s responses to care plan for the achievement of realistic client-centered outcomes and revises plan as required
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principals and established teaching materials to provide health teaching to clients and families
    • Analyses and incorporates current research findings into clinical practice (e.g.best practices, products & procedures)
    • Sanitization of equipment and instruments according to established regulations, policies and procedures.

    Resource Management

    • Refers clients to appropriate levels of nursing providers, support providers and to other disciplines (e.g., ET, Leg Ulcer specialist, Physician, etc.)
    • Documents rationale for referrals, for increase/decrease in visits
    • Provides cost-effective, organized, outcome directed care on an ongoing basis (e.g., supplies, visit utilization)
    • Demonstrates effective time management, planning and organizing of days’ work activities
    • Promotes client-directed care, self-care and independence through teaching

    Leadership

    • Works collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for other care providers, staff, and students

  • Qualifications
    • Licensed as a Registered Practical Nurse in the province of Ontario
    • Advanced Foot Care Certificate, the applicant must be willing to obtain the certificate with support from Carefor
    • Experience in foot care clinics preferred
    • Previous community experience will be considered an asset
    • Minimum of nine (9) months to one (1) year of related experience required

    • Implements the care plan based on the nursing assessment in collaboration with the physician, PSW’s and manager and review quarterly

The Opportunity: OICH Supervisor, Temporary

Under the direction of the Manager, Ottawa Inner-City Health, the Supervisor, Ottawa Inner-City Health is responsible for daily delivery of services provided by Client Care Workers (CCWs), including staff services issues. The Supervisor organizes and supports the orientation of new staff, identifies and follows up with job performance issues, collaborates with the OICH clinical team to ensure CCWs follow the care plan and all updates. The Supervisor ensures delegation of controlled acts are implemented as per regulation, and that CCWs are trained to provide the level of care. The Supervisor participates in client case conferences, works with the Professional Practice Leader to provide ongoing professional development, training, coaching, and assessments. The Supervisor completes regular performance reviews. Job Description
  • What You Will Do:
    • Oversees and assists the Manager with the overall clinical operations
    • Supports the Manager to oversee the Carefor CCWs assigned to Ottawa Inner-City Health
    • Acts as a resource, support and mentor to the Ottawa Inner-City Health staff and provides coaching, education, and instruction to encourage staff to attain individual and operational performance objectives.
    • Completes performance reviews on assigned Ottawa Inner-City Health Services staff.
    • Identifies and discusses with the Manager employee issues which require disciplinary action and ensures documentation is complete to support the disciplinary process.
    • Contributes to human resources management, including labor and employee relations, discipline, health and safety, continuing education, performance development, recognition of staff and orientation, coaching, and delegation as required.
    • Assists Manager in employee recruitment process, anticipates and monitors staffing needs and initiates appropriate action to obtain/maintain an adequate compliment of staff
    • Collaborates with the Client Servies Coordination team for staffing needs and ensures to communicate with OICH for any coverage issue and replacement plans.
    • Investigates and evaluates client and staff concerns/complaints and implements appropriate resolutions
    • Understands, promotes, and communicates the value, purpose, and relationship of the Ottawa Inner-City Health program to Carefor
    • Evaluates processes and implements strategies to increase efficiency and continuously promote quality improvement.
    • Develops procedures for all business practices in Ottawa Inner-City Health, and evaluates and revises policies and procedures on an ongoing basis to reflect changes in practice and requirements from funder or partner organization
    • Analyzes data to determine staffing complement in collaboration with the Manager
    • Works in collaboration with the OICH team and client/family to develop the initial Client Care Plan, revises as required, and completes Safety Assessments
    • Completes interim reports on clients every six months, and as requested by funder or partner organizations
    • Ensures that the quality of client care delivery is consistent with the standards of practice and regulatory requirements of the College of Nurses of Ontario (CNO) and any other governing regulatory body, as well as with the mission, vision, values and strategic plan of Carefor
    • Meets the contractual terms set forth by external/funding partners and maintains communication with these partners as needed
    • Participates in clinical call rotation
    • Assists with operation of the program in the Manager’s absence
    • Makes recommendations regarding CCW placement and schedules
    • Operates within and monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, regulatory standards of the CNO, practice standards, Carefor Policies & Procedures, and the OICH policies and procedures

  • Qualifications
    • Registered Practical Nurse, required
    • 3+ years community experience in nursing or Ottawa Inner-City Health; an asset
    • Demonstrated supervisory experience and the ability to delegate tasks and regulatory controlled acts as required
    • Proficiency in MS Office Programs and Database Management
    • Strong observation, assessment and problem-solving skills
    • Highly developed organizational and interpersonal skills
    • Excellent communication skills and an understanding of effective communication methods when dealing with a culturally diverse and vulnerable population

    • Schedules service providers for client visits in areas assigned by the manager.
    • Provides work schedules to staff that meet client needs and maximize continuity of care.

    • Provides complete, relevant assessment of the resident and describes and/or documents finding according to documentation procedures.

  • What You Will Do:

    Client-centred support and assistance with activities of daily living (ADLs), personal care and other care needs assigned. Duties will be carried out under the formal delegation of a registered nursing professional, in accordance with organizational policies, applicable legislation, and within the scope of training and competence. Responsibilities may include, but are not limited to the following:

    • Personal Care & ADLs: Assist clients with bathing, grooming, dressing, toileting, mobility, and safe transfers (including use of lifts and mobility aids).
    • Health Monitoring: Observe and report changes in client condition to the supervising nurse or care team.
    • Medication Assistance: Provide medication reminders or assistance as per care plan.
    • Environmental Support: Perform light housekeeping, laundry, and meal preparation to maintain a safe and clean environment.
    • Social & Emotional Support: Engage clients in conversation and activities to reduce isolation and promote well-being.
    • Documentation & Communication: Accurately document provided care and communicate effectively with clients, families, and team members.
