The Opportunity: Nursing Supervisor, Temporary

Reporting to the Manager of Allied Health Services, the Nursing Supervisor, @Home Programs, is responsible for the case management and nursing care of clients transitioning out of the hospital as part of the Carefor’s @Home bundled care programs. The Nursing Supervisor, @Home Programs is responsible for managing the wrap-around care that is coordinated and delivered to clients admitted to the program, with support from members of the care team. The Nursing Supervisor is responsible for conducting InterRAI-HC assessments, designing care plans, engaging with other members of the care team (e.g., social workers, PTs, OTs, PTA/OTA, nutritional specialists, and speech language pathologists), and delegating tasks to personal support workers assigned to deliver care. The Nursing Supervisor is also responsible for providing clinical support, identifying and following up with PSW job performance issues, participating in client case conferences, providing ongoing training and assessments, and providing feedback on performance.

As a critical resource to the @Home Program, the Nursing Supervisor will support the care team and clients in providing safe, client-centred care by modeling practice standards, defining care processes and procedures, and collaborating in the design of personal support workers’ education and training. Striving for excellence in quality client care, the position provides leadership and supports the implementation of evidence-based professional practice projects, programs, and initiatives for Carefor’s @Home Program(s).

Job Description
  • What You Will Do:
    • Leads the care management and home care assessment responsibilities of clients transitioning out of hospital within the @Home Program;
    • Acts as primary nurse to clients within the @Home Program in the delivery of nursing care;
    • Acts as a resource, support and mentor to the @Home Program care team, providing coaching, education, and instruction to encourage individual and operational performance objectives. Encourages and models critical clinical inquiry that cultivates evidence-informed practice;
    • Supports the performance management process with assigned staff and engages with appropriate managers regarding any performance issues which require disciplinary action;
    • Contributes to human resources management including employee relations, progressive discipline, health and safety, continuing education, performance development, recognition of staff and orientation, as required;
    • Assists the Manager in identifies areas for recruitment, anticipates and monitors staffing needs and initiates appropriate action to obtain/maintain an adequate compliment staff for the @Home program;
    • Investigates and evaluates client and staff concerns/complaints and implements appropriate resolutions, as required; and
    • Provides leadership coverage of the @Home Team as required.
    • Leads by example and promotes continuous quality improvement;
    • Understands, promotes, and communicates the value, purpose, and goals of the @Home Program;
    • Working with the Allied Health Services Manager in the evaluation and revision of @Home Program policies and procedures, based on changes in practice and requirements from funder or partner organization;
    • Ensures that the quality of client care delivery is consistent with Carefor’s mission, vision, values and strategic plan;
    • Meets the contractual terms set forth by external/funding partners and maintains communication with these partners as needed;
    • Assists with operation of the program; provision of direct client care as required;
    • Collaborate to address quality, safety and risk management issues, including the creation of professional development and performance improvement plans to address shortfalls;
    • Contribute to the improvement of client outcomes (quality of life and quality of care) based on best available evidence and efficient utilization of resources;
    • Contribute to business development initiatives, as required;
    • Support the @Home Program team through regular meetings, orientation, mentorship, consultation, and guidance with complex clinical situations. Participate on internal and external case conferences or meetings as required; and
    • Operates within as well as monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures.

    Working Conditions:

    • Combined regular office environment with some flexibility of remote work.
    • A vehicle for travel within the Ottawa region to support Nursing staff working in various community settings (e.g. individual homes, retirement homes, Carefor clinics) will be required.
    • Ability to work flexible hours to support client needs, may be required.
    • Leadership On-Call rotation with evening, weekend, and statutory holiday coverage.
  • Qualifications
    • Registered Nurse (RN) in good standing with the College of Nurses of Ontario is required.
    • Three (3) years recent experience working within the adult home health care sector with related experience to support the provision of restorative care to older adults.
    • Knowledge of the related regulatory and legislative requirements within Ontario is required.
    • Certification in clinical specialties and chronic disease management is preferred.
    • Ability to promote and facilitate evidence-based practice within a community setting with demonstrated expertise and ability to apply, teach, coach, and mentor individuals and teams.
    • Superior communication skills and ability to communicate effectively to direct and motivate others while maintaining a persuasive and credible presentation style at all levels of the organization.
    • Demonstrated effective critical thinking, analytical, and problem-resolution skills.
    • Excellent organizational, planning, and time management skills.
    • Proficiency in using Microsoft Office applications (e.g., Outlook, Word, Excel, etc.).
    • Experience in a unionized environment is an asset.
    • Bilingualism (French/English) considered an asset.

  • What You Will Do:

    As a Registered Practical Nurse in Carefor’s Visit Nursing Program, you will make a difference in the lives of your clients in the community by utilizing your excellent communication skills to obtain relevant information from your clients and their families.   • What You Will Do:
    • Assist or provide care according to the Resident Care Plan. Assistance may need to be provided on a short or long term basis to individual residents including but not limited to personal care, lifts and transfers, observing nutritional status, light housekeeping, assist in taking prescribed medication, etc…
    • Observe resident for changes in behaviour or signs or symptoms of decreasing health status and report to Care Team
    • Chart care given and give verbal/written report to RPN or other team members on any unusual behaviour and observations made with respect to clients/residents
    • Evaluate the individual’s need for change in the personal support care plan and make recommendations to RPN or designate.
    • Contributes to the development of the personal support care plan by reporting, to the RPN or designate, all observations or information obtained from the resident and/or family regarding needs and the effect of the care he/she provided.
    • Plans and organizes each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment.
    • Completes documentation/paperwork according to Carefor policies and procedures
    • Reviews all forms of documentation and communication in order to remain current on residence operations and resident care. This may include reviewing care notes, e-mails, memos, communication book, and receiving verbal updates
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard

The Opportunity: Physiotherapy Assistant- Permanent

The Physiotherapy Assistant (PTA) provides clinical care for clients under the direction and supervision of the Physiotherapist and Occupational Therapist. The PTA works to actively rehabilitate each client to achieve his/her maximum functional level through planned and supervised therapeutic programs.     • Assist physiotherapists with the provision of individual and group therapeutic exercise programs
    • Learn programs of care from the Physiotherapist and provide care to individual clients based on the frequency specified in the program of care
    • Lead a group of clients in a specified group exercise program at the frequency and time specified by the Physiotherapist
    • Monitor the client’s ability and accommodate any changes from baseline. Report back to the Physiotherapist the need to change/adjust the program.
    • Assist physiotherapists with the provision of application and removal of ice, heat and various therapeutic modalities
    • Understand and follow all directions and specifics related to the application of modalities
    • Ensure client safety at all times
    • Monitor client’s progress and report back to the Physiotherapist
    • Complete written reports as required
  • Qualifications
    • Two Year Physiotherapy Assistant Diploma from an institution recognized by the College of Physiotherapists of Ontario or an undergraduate degree in human kinetics.
    • Minimum of 2 years recent related experience in adult physical medicine gained in a community, rehabilitation, or long-term care setting.
    • Skills:
    • Demonstrated understanding of the principles of rehabilitation and the rehabilitation process
    • Knowledge of anatomy, common injuries, and diseases, basic stretching and strengthening exercises
    • Possesses strong critical thinking and problem-solving skills

  • What You Will Do:

    Purchasing Key Responsibilities

    • Responsible for purchasing activities, including preparing detailed purchase orders, ordering, receiving, shipping, stocking, tracking, verifying, managing forms and supplies inventory etc.
    • Conduct cost-effective procurement to acquire goods at competitive rates.
    • Research vendors and products carefully based on quality, prices and delivery terms.
    • Evaluate vendors based on their business history and supply capabilities.
    • Follow-up with suppliers to schedule deliveries, resolve material discrepancies, monitor backorder items, and handle supply returns.
    • Purchase order (PO) management, ensuring all key reports are generated. Compile and match purchase orders with invoices and packaging lists/pick lists.
    • Execute accurate and timely documentation gathering (packing slips/PO/receipt/invoice) to facilitate invoice processing.
    • Maintain supplier agreements terms and conditions related to quality requirements, prices, payment terms, delivery time, freight, and other requirements to ensure fiscal benefit for the company.
    • Assess the quality of goods at every step of the procurement process.
    • Maintain corporate records of suppliers, price lists and shipping instructions.
    • Produce and maintain authorized vendor list with ordering process and product information.
    • Track and maintain the inventory of any physical goods received or sent.
    • Produce, maintain, and distribute authorized supply lists for department orders, separating into categories (i.e. office supplies, nursing supplies, PPE etc.)
    • Assist nursing clinics in periodic counts and best practices for ordering, min max etc.
    • Assist other departments in training and understanding corporate procurement policies and procedures including purchase order approval process.
    • Prepares activity reports as required including evaluating product usage for bulk ordering and forecasting inventory levels.
    • Arranging and executing periodic inventory counts – annual and/or quarterly as needed.
    • Ensures building access cards records are up to date with periodic report investigations. Creates, deletes and updates building access cards, employee ID cards.
    • Participates in orientation of new employees by creating Security Pass cards, organizing, and distributing supplies and equipment packages to help ensure each employee has the necessary supplies/equipment to fulfill their role.
    • Receive and track outstanding Carefor office/nursing equipment and supplies from terminating employees and restock as needed, ensuring back up stock is available.
    • Coordinates and maintains the annual maintenance schedule for Allied Health and nursing equipment.
    • Ensure medical waste is packaged according to Stericycle standard.
    • Recycle toners on regular basis, following IT agreements with printer companies.
    • Recycle batteries on a regular basis.
    • Work with other programs, sites to share and use excess materials.
    • Coordinates and maintains the Material Safety Data Sheets for all supplies and equipment that are of chemical and/or biological hazard.
    • Completes all tasks and functions in compliance with Occupational Health and Safety Legislation, regulations, and Carefor policies and procedures.
    • Maintains up-to-date procurement processes and procedure guidelines.
