  • What You Will Do:

    As a Registered Practical Nurse in Carefor’s Visit Nursing Program, you will make a difference in the lives of your clients in the community by utilizing your excellent communication skills to obtain relevant information from your clients and their families. You will implement nursing strategies or interventions according to the established care plan in consultation with the client and/or caregiver and the health care team. You will utilize adult learning principals and teaching materials to provide guidance to clients and their caregivers.     • Assess and treat clients with a wide range of acute and chronic conditions
    • Test and measure the functioning of the musculoskeletal, neurological, pulmonary, and cardiovascular systems.

  • Qualifications

    • Registration with the College of Physiotherapy of Ontario
    • Excellent time management and organizational skills and ability to work independently.
    • Excellent oral and written communication skills
    • Strong skills in critical thinking, decision-making, and analysis
    • The ability to work collaboratively with case managers, clients, families, and other caregivers to provide service.
    • Possession of a valid driver’s license (G2 level) and use of a reliable vehicle.
    • 2–5 years Clinical practice experience preferred
    • ADP Authorizer preferred
    • Experience in community health is an asset

  • What You Will Do:
    • Maintain a clean, safe, home – like environment
    • Collect garbage from hallways, lounges and other areas and deposit in the designated area.
    • Services and maintains hot water boiler system to the limitation of the certification of the incumbent
    • Services and maintains heating, ventilation and air conditioning equipment including pumps, valves, motors, fans etc. as per preventative maintenance schedule to the limitation and certification of the incumbent
    • Repairs and maintains custodial and food services equipment
    • Tests and controls water systems and related equipment
    • Operates boilers and performs preventative maintenance on boilers and ancillary equipment and assist with preventative maintenance schedule
    • Performs pipe fitting
    • Service and install window air conditioning units
    • Services and maintains pneumatic controls
    • Performing repairs and maintains building, grounds equipment, and residents personal items, which may be of an electrical, mechanical, or carpentry in nature (woodworking, drywall, painting etc.)
    • Performing required floor care i.e. strip, seal, wax, buff, and damp mop floors
    • Carry out regular fire inspections as assigned
    • Operate light equipment i.e: lawnmowers, hedge trimmers, snow blower etc. according to operating instruction and report any malfunction to Manager
    • Keeping sidewalks, door entrances, stairways, roadways, parking areas, etc. free of hazards ( e.g. snow, ice, garbage, sand etc)

  • What You Will Do:
    • Assist or provide care according to the Resident Care Plan. Assistance may need to be provided on a short or long term basis to individual residents including but not limited to personal care, lifts and transfers, observing nutritional status, light housekeeping, assist in taking prescribed medication, etc…
    • Observe resident for changes in behaviour or signs or symptoms of decreasing health status and report to Care Team
    • Chart care given and give verbal/written report to RPN or other team members on any unusual behaviour and observations made with respect to clients/residents
    • Evaluate the individual’s need for change in the personal support care plan and make recommendations to RPN or designate.
    • Contributes to the development of the personal support care plan by reporting, to the RPN or designate, all observations or information obtained from the resident and/or family regarding needs and the effect of the care he/she provided.
    • Plans and organizes each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment.
    • Completes documentation/paperwork according to Carefor policies and procedures
    • Reviews all forms of documentation and communication in order to remain current on residence operations and resident care. This may include reviewing care notes, e-mails, memos, communication book, and receiving verbal updates

A Registered Practical Nurse with Carefor is an integral part of the Carefor Health Team and is responsible for the provision and management of nursing care to individuals and/or families in the community, as well as teaching and delegating tasks to different members of the Carefor team.

This residence is a secure bungalow in the heart of Richmond Ontario and is designed to help residents feel as though they are living in a home away from home. The living room, kitchen and dining area are the central hub of the residence with comfortable, familiar décor. Our highly trained staff are encouraged to wear regular clothes instead of scrubs or uniforms, this encourages our residents to feel even more at ease.   • What You Will Do:

    Primary Responsibilities include:

    As the RPN at the home you will function as the leader of the resident care team. Utilizing your professional knowledge to assess clients, create care plans, administer medication and monitor/ evaluate the well being of the clients. In the absence of the manager you are in charge to ensure the residence comfort and well being and that policies and procedures are being followed.

  • Qualifications

    In our fast-paced, responsive, and caring environment, we are looking for a self-starter who has the experience and ability to jump in on day one with energy and enthusiasm.

    The following items describe important experience and competency areas you have demonstrated in your current or previous roles in order to be successful in this role:

    • You are registered in good standing with the College of Nurses in Ontario (CNO)
    • You have excellent high-tech skills (with IV restart) or a passion to be trained in these skills
    • You possess a valid driver’s license and use of a reliable vehicle
    • You have excellent documentation skills
    • You have the ability to work collaboratively as a member of an interdisciplinary practice
    • You have excellent problem solving skills and the ability to adapt nursing to independent practice
    • You take initiative and you are passionate about caring for your clients
    • Admissions and Discharges of clients as required

    If you have the following, it would be considered an additional asset:

    • Ability to converse in both official languages (English/French)
    • Experience in palliative care
    • You have LEAP training or are willing to take part as a requirement to palliative care provision
    • Experience in community health care
    • Current skills in Foot Care
