The Client Services Coordinator works as part of a team to implement Carefor’s approved or prescribed processes to ensure every client visit is properly scheduled, every service provider’s caseload is properly assigned, and client expectations are met. The Client Services Coordinator also maintains accurate documentation and communications with clients, service providers, and other users. A Client Services Coordinator may have to manage client expectations to ensure the expected service level is achieved. Please note that candidates may be selected with the use of interview and assessment processes.

Under the supervision of the Care Team Supervisor, the Registered Practical Nurse (RPN) is responsible for the provision and management of cost effective, quality nursing care to residents of the Pembroke Civic Complex. RPN will assume a leadership role by teaching, delegating, and providing guidance to different levels of workers within the care team. Care is provided within the framework of quality & risk management, Carefor policies and procedures, and the standards and scope of practice set by the College of Nurses of Ontario (CNO).

Under the supervision of the Care Team Supervisor, the Personal Support Worker (PSW) provides personal support and homemaking services to individuals. Care is provided within the framework of policies, procedures, standards and quality & risk management as established by Carefor Health and Community Services and the management of the Pembroke Civic Complex. The PSW collaborates with members of the health care team in meeting the needs of individuals to attain an optimal level of independence and wellbeing. A PSW is responsible to work within his/her scope of practice, performing the care and activities for which they have the necessary training and for which they are competent. The PSW is to inform the manager or designate if they require additional training with tasks.
