The Opportunity: Registered Dietitian – Temporary

Is responsible for the provision and management of cost-effective Dietetic care to individuals and/or families in the community. Care is provided within the framework of quality & risk management and the standards and scope of practice set by Carefor and the provincial regulatory body (CDO) for the Dietitian. Is responsible for the assessment, treatment, consultation, and counseling of clients and their families in the community by developing a Client Care Plan for each Client in accordance with the Client Service Plan and in consultation with the Client and, if applicable, the Caregiver; providing Dietetic Interventions to Clients in accordance with the Client Care Plan; handling and caring for Equipment and Supplies relating to the delivery of Dietetic Services at the Service Delivery Location; communicating with the CCAC through the maintenance of Client Records; developing a teaching plan that will enable the Client to achieve the Care Plan Goals.

Job Description
  • What You Will Do:
    • Schedules service providers for client visits in areas assigned by manager
    • Provides work schedules to staff that meet client needs and maximize continuity of care.
    • Keeps up to date with daily changes such as staff availability, changes in client service
    • authorizations and/or care plans
    • Ensures client and service provider information is accurate and up to date in the
    • scheduling database
    • Provides timely information, through the use of communication technology (telephone,
    • fax, e-mail, voicemail), to field staff, services providers, co-workers, Managers and
    • Supervisors, clients and customers
    • Verifies visit confirmations from field staff and processes in a timely manner to be
    • processed by the billing department
    • Verifies payroll information for field staff in a timely manner ensuring accuracy of hours,
    • mileage, stats and premiums.
    • Contacts LHIN regarding discrepancies in frequency, authorization for extra visits or not
    • seen/ not found visits, extensions to planned review dates
    • Works collaboratively as part of a team willing to assist others and mentor new staff
    • Contributes to the principles of customer-focused service and continuous quality
    • improvement
    • Functions and completes all duties in compliance with Occupational Health and Safety
    • legislation, regulations, and Carefor policies, procedures, and standards
  • Qualifications
    • Registration with the College of Dietitians of Ontario
    • BSc
    • Driver’s License and a reliable vehicle that is insured for business use
    • 3 to 5 years Clinical practice experience preferred

The Opportunity: Supervisor- Client Services Services, Temporary

About the role: Under the direction of the respective Program Managers, Client Service Supervisors are responsible for overseeing the daily staffing, scheduling and coordination of client care visits for three programs, namely nursing, allied health and personal support services as well as for Carefor’s reception desk. In collaboration with the Training & Process Improvement Coach, Supervisors create an engaging orientation environment, encourage and support ongoing training as required, ensure expectations and accountabilities are understood and complete regular performance reviews. They are responsible for the standard work and associated operational processes associated with program coordination. Supervisors, in collaboration with the respective Program Managers and subject matter experts, develop procedures for all program business practices understanding that changes in one department can create system change in other departments. Supervisors also embrace enabling technology to further innovate and build efficiency and effectiveness for program improvement

Job Description
  • What You Will Do:
    • Oversees and assists the Program Manager with the overall operation of the program’s clerical staff.
    • Understands and demonstrates the value of coaching in staff performance.
    • Seeks opportunities to support and mentor staff in achieving optimal performance.
    • Provides guidance and advice to Coordinators when they identify operational challenges.
    • Meets with staff at least every 6 months to review performance goals, staff achievements and areas of further development.
    • Evaluates new employee’s orientation and probationary periods to determine their progression through this phase and if additional education/mentoring and coaching or additional performance improvement planning is required.
    • Collaborates with the Training & Process Improvement Coach, Supervisor(s) and Manager, Client Services to provide leadership to the team and to create an environment conducive to respectful, empowering working relationships.
    • Completes biannual performance reviews on all clerical staff.
    • Operational and Organizational Responsibilities:
    • Understands leadership as creating space for empowerment; leads by example.
    • Committed to managing using data and promotes continuous quality improvement.
    • Evaluates processes and implements strategies to increase efficiencies, using a lean approach.
    • Develops procedures for all business practices in client services, and evaluates and revises policies and procedures on an ongoing basis based on changes in practice and requirements from funder or partner organization.
    • Analyzes data to determine staffing complement in collaboration with the Manager.
    • Maintains effective and frequent communication with the client services team to ensure staff scheduling is meeting service delivery needs.
    • Oversees the development and maintenance of the master rotation for all three programs, based on management requirements.
    • Provides staff work schedules that meet employee and client needs while maximizing continuity of care, and works within the provisions of collective agreements, Labour Standards and funder or partner organization guidelines.
    • Grants time off in accordance with the collective agreement, while ensuring adequate staff are scheduled to meet operational requirements.
    • Fosters employee engagement by inviting feedback and being transparent.
    • Monitors time away from work such as sick, stats, and overtime and vacation credits for staff requiring time off based on reports from payroll and human resources.
    • Arranges the preceptorship of students for nursing and allied health in collaboration with Program Managers.
    • Reviews and adapts job descriptions to meet the ever changing needs of the programs/services.
    • Liaises with the client services team and other departments to ensure integrated systems and processes.
    • Health and Safety Responsibilities:
    • Operates within and monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures.
    • Participates in health and safety training including WHMIS and applies this knowledge.

The Opportunity: Part Time Temporary Personal Support Worker

  • What You Will Do:
    • Assist or provide care according to the Resident Care Plan. Assistance may need to be provided on a short or long term basis to individual residents including but not limited to personal care, lifts and transfers, observing nutritional status, light housekeeping, assist in taking prescribed medication, etc…
    • Observe resident for changes in behaviour or signs or symptoms of decreasing health status and report to Care Team
    • Chart care given and give verbal/written report to RPN or other team members on any unusual behaviour and observations made with respect to clients/residents
    • Evaluate the individual’s need for change in the personal support care plan and make recommendations to RPN or designate.
    • Contributes to the development of the personal support care plan by reporting, to the RPN or designate, all observations or information obtained from the resident and/or family regarding needs and the effect of the care he/she provided.
    • Plans and organizes each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment.
    • Completes documentation/paperwork according to Carefor policies and procedures
    • Reviews all forms of documentation and communication in order to remain current on residence operations and resident care. This may include reviewing care notes, e-mails, memos, communication book, and receiving verbal updates
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents
  • Qualifications
    •  Secondary school education preferred
    • Personal Support Worker or Health Care Aide certificate
    • Current Standard First Aid Certificate, required
    • Current CPR level C Certificate, required
  • Experience
    • At least 1 year experience working with elderly or ill individuals.
    • Experience working with seniors who have dementia, Alzheimer’s, and mental health illnesses
    • Experience working in a retirement or nursing home facility

  • What You Will Do:
