The Opportunity:Manager, Transitions Programs Permanent

Reporting to the Director Client Care, the Manager Transitions Programs ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, community support services, primary care and acute care teams.  Together, they create personalized plans that improve outcomes and ease the transition to home. 

The Manager, Transitions Programs provides operational leadership to transitional programs and the interdisciplinary teams that will be providing holistic care to the clients in the community. You are passionate about informing and shaping transitional programs from hospital to the community and being at the forefront of healthcare innovation in Ontario. You will provide exemplary leadership and operational management for interdisciplinary team(s) including nursing, therapies, social work, dietitian, PSWs and community support services care providers.  You will ensure excellence in the provision of client care, the achievement of corporate/program objectives; planning and operations; quality management and improvement; participation in special projects, corporate/research initiatives and other activities that support and advance organization goals. This role will have accountabilities in operational and professional practice excellence; creating a healthy workplace that maximizes the potential of all members of the team.

Job Description
  • What You Will Do:
    • Provide clinical practice leadership, coaching and oversight management to ensure the safe, effective and efficient provision of care excellence to all clients and family members for transitions programs.
    • Develop, implement, manage and review short- and long-term plans for the clinical area(s) which are in aligned with organizational goals and strategies; identifies and prioritizes time sensitive initiatives.
    • Develops and implements day-to-day operating policies, systems and procedures to contribute to organizational effectiveness, quality improvement, and innovation.
    • Develop staff utilization plans to ensure adequate level/mix of human resources to execute plans and achieve required outcomes.
    • Identifies and manages employee issues/concerns.
    • Manage, motivate and provide professional/clinical leadership to interdisciplinary teams; ensuring ongoing practice and professional development of a cohesive team;
    • Apply structured change management methodologies (e.g., ADKAR, Kotter’s 8-Step, etc.) to guide transitions and foster engagement.
    • Mentor clinical staff and contribute to workforce development initiatives.
    • Collaborate with the Professional Practice and human resources to ensure proper onboarding of new client care employees.
    • Promote external and internal sharing of best practices to increase efficiency, reduce costs and improve quality.
    • Implement oversight mechanisms to monitor, measure, and evaluate compliance with regulated standards of care.
    • Conduct regular performance appraisals, in a manner that is fair and consistent.
    • Lead by example to plan and implement continuous quality improvement and other initiatives in the clinical area(s) by creating an environment where innovation and creativity are encouraged.
    • Addresses adverse events, complete risk investigation and develop risk management strategies.
    • Adopts Diversity, Inclusivity, and Equity in the delivery of client care services.
    • On-call as required for programs support
  • Qualifications
    • Registered healthcare professional licensed to practice in Ontario and member of one of the regulatory colleges such as but not limited to the professions of RN, OT, PT, SW, DT, Kinesiologist, Psychologist, Behavioralist; required.
    • Undergraduate preparation in a Healthcare related Discipline, required.
    • Master’s prepared; asset
    • Minimum 3–5 years of experience in management; preferred.
    • Knowledge of the related regulatory and legislative requirements within Ontario; required.
    • Demonstrated business acumen to development and implementation new ventures.
    • LEAN Greenbelt, PROSCI, or PMP certificate; asset.
    • Demonstrated experience in change management, project leadership, and interdisciplinary collaboration; asset.
    • Proven understanding of clinical service delivery in a homecare environment.
    • Excellent communication and leadership skills.
    • Good command of Windows, Microsoft Office 365 (Outlook, Word, Excel, Teams, etc.).
    • Experience supporting quality and risk management initiatives.
    • Demonstrated ability to work independently and to take initiative.
    • Understands and uses effective communication methods/tools adopted by the organization to communicate effectively influence change, direct and motivate others.
    • Adapts to changes in the role by acquiring the appropriate knowledge and skills
    • Ability to promote and facilitate data driven, evidence-based practice decision-making within a community setting with demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams and drive results.

    The hiring range for this position is $95,101.50 to $114,016.50 per year, which reflects the expected starting salary based on qualifications and experience. The Opportunity: Supervisor- Nursing Services, Home & Clinical Care Permanent

Reporting to the Manager of Nursing Services, this position is responsible for the clinical supervision of nursing staff in the Ottawa region. In collaboration with the Professional Practice Lead (Nursing), the Nursing Supervisor is responsible for orienting new staff, identifying and following up with job performance issues, providing clinical support, participating in client case conferences, providing ongoing training and assessments, and completing regular performance reviews.

As a critical resource to the Nursing team, the Nursing Supervisor will support the Nursing Team in providing safe, client-centered care by modeling practice standards, defining care processes and procedures, and collaborating in the design of Nurses’ education and training.

Striving for excellence in quality client care, the position provides leadership and supports the implementation of evidence-based professional practice projects, programs, and initiatives for the Nursing Team.

Job Description
  • What You Will Do:
    • Assists the Manager of Nursing Services with the overall operation and performance of the Nursing program.
    • Acts as a resource, support and mentor to the Nursing Team and provides coaching, education, and instruction to encourage individual and operational performance objectives. Encourage and model critical clinical inquiry that cultivates evidence-informed practice.
    • Completes Performance Appraisals with assigned nurses and engages with the Manager of Nursing and Professional Practice Lead regarding any performance issues which require disciplinary action.
    • Contributes to human resources management including labour and employee relations, discipline, health and safety, continuing education, performance development, recognition of staff and orientation, as required.
    • Assists the Manager in recruitment, anticipates and monitors staffing needs and initiates appropriate action to obtain/maintain an adequate compliment of nursing staff, as required.
    • Investigates and evaluates client and staff concerns/complaints and implements appropriate resolutions, as required.
    • Provides leadership coverage of the Nursing Team as required (i.e. Manager of Nursing).
    • Oversees nursing services in collaboration with the Nursing Manager (i.e. foot care, shift nursing program, clinics).
    • Leads by example and promotes continuous quality improvement.
    • Understands, promotes, and communicates the value, purpose, and goals of the nursing program.
    • Working with the Nursing Manager in the evaluation and revision of nursing policies and procedures, based on changes in practice and requirements from funder or partner organization.
    • Ensures that the quality of client care delivery is consistent with Carefor’s mission, vision, values and strategic plan.
    • Meets the contractual terms set forth by external/funding partners and maintains communication with these partners as needed.
    • Assists with operation of the program. Provision of direct client care as required.
    • Collaborate to address quality, safety and risk management issues, including the creation of professional development and performance improvement plans to address shortfalls.
    • Contribute to the improvement of client outcomes (quality of life and quality of care) based on best available evidence and efficient utilization of resources.
    • Monitor federal, provincial, and other relevant legislation/regulations for compliances and collaborate on strategies to address opportunities for improvement.
    • Contribute to business development initiatives, as required.
    • Support the Nursing Team through regular meetings, orientation, mentorship, consultation and guidance with complex clinical situations. Participate on internal and external committees as required.
    • Operates within as well as monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures.
  • Qualifications
    • Registered Nurse in good standing with the College of Nurses of Ontario is required.
    • Three (3) years recent experience of working within the Adult Home Health Care sector with related expert practice skills to support the provision of Nursing care to older adults.
    • Knowledge of the related regulatory and legislative requirements within Ontario is required.
    • Certification in clinical specialties and chronic disease management is preferred.
    • Ability to promote and facilitate evidence based practice within a community setting with a demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.
    • Superior communication skills and ability to communicate effectively to direct and motivate others while maintaining a persuasive and credible presentation style at all levels of the organization.
    • Demonstrated effective critical thinking, analytical, and problem-resolution skills.
    • Excellent organizational, planning and time management skills.
    • Proficiency in using Microsoft Office applications (e.g., Outlook, Word, Excel, etc.).
    • Experience in a unionized environment is an asset.
    • Bilingualism (French/English) considered an asset.

The Opportunity: Clinical Supervisor, Permanent

Carefor is seeking a dedicated and skilled Registered Practical Nurse (RPN) to support our Adult Day Programs (ADP) and Community Support Services (CSS). In this dynamic role, you will provide direct client care, monitor health and well-being, administer medications, and conduct assessments using tools such as the RAI-CHA. Job Description
  • What You Will Do:
    • Oversees and assists the Manager with clinical operation of the ADP and CSS programs
    • Acts as a resource, support and mentor to the ADP and CSS staff by leading and coaching staff to attain individual and operational performance objectives
    • Ensures Carefor policies & procedures affecting the delivery of ADP and CSS programs are implemented by staff
    • Participates in annual performance reviews on ADP and CSS staff
    • Identifies and discusses with the Manager client care issues
    • Direct client care and assessment of needs
    • Support the practice involving medication management protocols
    • Participate in the development, implementation, and updates of care plans
    • Client monitoring, observe and document and physical or mental health concerns and report to the most appropriate person (Manager, Supervisor, Primary Care Provider)
    • Communicate with clients, family members, and other care providers (i.e. Alzheimer’s Society, OHAH), to ensure the best possible care and address any concerns or issues
    • Maintain accurate and timely records of client care, using electronic documentation systems (AlayaCare) and adhering to established standards
    • Complete RAI-CHA assessments in the client’s home
    • Acts within the function of supporting non-regulated staff in a professional practice lead capacity
    • Understands, promotes, and communicates the value, purpose, and relationship of Community Support Service programs to Carefor as part of its strategic plan
    • Promotes continuous quality improvement by contributing to the implementation and evaluation of strategies designed to increase the provision of quality care
    • Works in collaboration with client/family to develop the Client Care Plan and complete safety assessments, updates them as required, and communicates changes with the ADP and CSS team.
    • Investigates, evaluates and supports the resolution of identified client and staff concerns & complaints
    • Makes recommendations for continuous improvement relating to Carefor’s overall objectives, policy, and procedures.
    • Decisions may require review or escalation when costs, potential liability, reputational risk and/or an impact on objectives or staff are involved.
    • Direction is generally received for outliers to organization’s policies, objectives, and values.
    • Operates within and monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures
    • Participates in health and safety training including WHMIS and applies this knowledge
    • Works in collaboration with Management and Human Resources to support return to work plans for staff taking part in modified work
  • Qualifications
    • A Registered Practical Nurse (RPN), licensed to practice in Ontario, in good standing, is required
    • A minimum 1-year in dementia care programs such as Adult Day Programs is preferred
    • A minimum of 3 years community experience in Nursing or Personal Support Services
    • Demonstrated supervisory experience and the ability to delegate tasks as required, preferred
    • Demonstrated an understanding of geriatric psychiatry, geriatrics, and dementia care with highly developed organizational skills
    • Demonstrated understanding of College of Nurses of Ontario Standards of Practice
    • Excellent interpersonal and verbal/written communication skills
    • Excellent computer skills/knowledge and familiarity with CIMS and Alayacare systems
    • Ability to work collaboratively with internal and external stakeholders
    • A vehicle and a valid Ontario Driver’s license are required
    • Ability to manage coinciding demands
