Carefor pleased to be part of successful Ottawa Health Team application

Ottawa – Carefor Health & Community Services is very excited to be part of the successful Ottawa Health Team/Équipe Santé Ottawa application announced today by Minister Christine Elliott in Ottawa. The Ottawa Health Team/Équipe Santé Ottawa will be one of 24 teams in the province to implement this new model of organizing and delivering health care that better connects the people that require care and support with providers to improve health. It will integrate services from primary care, home care, community care, hospital care and public health to create a seamless system for the people that require care and support and their families and caregivers.
Carefor is one of 11 convening partners among the 60 local health and social services organizations that make up the Ottawa Health Team/Équipe Santé Ottawa. The organizational partners represent primary care, acute care, sub-acute care, long term care, supportive housing, home and community support, mental health and addictions support, palliative care, emergency services, public health and social services.
In its first year, the Ottawa Health Team/Équipe Santé Ottawa will focus on two initial populations, both of which require greater coordination of care, clearer pathways to support them and improved access to specialists: frail older adults and adults with moderate to complex mental health and/or addictions issues.

We expect the Ottawa Health Team/Équipe Santé Ottawa to result in more efficient and seamless care for our clients.”
It is expected that the majority of Carefor’s services will be part of the Ottawa Health Team/Équipe Santé Ottawa’s scope. We are currently at the beginning of this transition and are learning the specific details of how the Ottawa Health Team/Équipe Santé Ottawa, and Carefor as part of it, will operate. Carefor is currently part of another submission for a health team in Eastern Ottawa.
