The Opportunity: Clinical Supervisor, Permanent

Carefor is seeking a dedicated and skilled Registered Practical Nurse (RPN) to support our Adult Day Programs (ADP) and Community Support Services (CSS). In this dynamic role, you will provide direct client care, monitor health and well-being, administer medications, and conduct assessments using tools such as the RAI-CHA. You will develop and update individualized care plans, maintain accurate documentation, and communicate effectively with families and care teams.
As a key member of the interdisciplinary team, you will also assist with activity coordination, participate in client case conferences with Ontario Health @Home Care Coordinators, and contribute to resolving service concerns. In collaboration with the Professional Practice Team, you will support staff orientation, training, performance follow-up, and reviews. Your work will be guided by evidence-based practices and data-informed decision-making to ensure high-quality service delivery and team performance.

Job Description

What You Will Do:
- Oversees and assists the Manager with clinical operation of the ADP and CSS programs
- Acts as a resource, support and mentor to the ADP and CSS staff by leading and coaching staff to attain individual and operational performance objectives
- Ensures Carefor policies & procedures affecting the delivery of ADP and CSS programs are implemented by staff
- Participates in annual performance reviews on ADP and CSS staff
- Identifies and discusses with the Manager client care issues
- Direct client care and assessment of needs
- Support the practice involving medication management protocols
- Participate in the development, implementation, and updates of care plans
- Client monitoring, observe and document and physical or mental health concerns and report to the most appropriate person (Manager, Supervisor, Primary Care Provider)
- Communicate with clients, family members, and other care providers (i.e. Alzheimer’s Society, OHAH), to ensure the best possible care and address any concerns or issues
- Maintain accurate and timely records of client care, using electronic documentation systems (AlayaCare) and adhering to established standards
- Complete RAI-CHA assessments in the client’s home
- Acts within the function of supporting non-regulated staff in a professional practice lead capacity
- Understands, promotes, and communicates the value, purpose, and relationship of Community Support Service programs to Carefor as part of its strategic plan
- Promotes continuous quality improvement by contributing to the implementation and evaluation of strategies designed to increase the provision of quality care
- Works in collaboration with client/family to develop the Client Care Plan and complete safety assessments, updates them as required, and communicates changes with the ADP and CSS team.
- Investigates, evaluates and supports the resolution of identified client and staff concerns & complaints
- Makes recommendations for continuous improvement relating to Carefor’s overall objectives, policy, and procedures.
- Decisions may require review or escalation when costs, potential liability, reputational risk and/or an impact on objectives or staff are involved.
- Direction is generally received for outliers to organization’s policies, objectives, and values.
- Operates within and monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures
- Participates in health and safety training including WHMIS and applies this knowledge
- Works in collaboration with Management and Human Resources to support return to work plans for staff taking part in modified work

Qualifications
- A Registered Practical Nurse (RPN), licensed to practice in Ontario, in good standing, is required
- A minimum 1-year in dementia care programs such as Adult Day Programs is preferred
- A minimum of 3 years community experience in Nursing or Personal Support Services
- Demonstrated supervisory experience and the ability to delegate tasks as required, preferred
- Demonstrated an understanding of geriatric psychiatry, geriatrics, and dementia care with highly developed organizational skills
- Demonstrated understanding of College of Nurses of Ontario Standards of Practice
- Excellent interpersonal and verbal/written communication skills
- Excellent computer skills/knowledge and familiarity with CIMS and Alayacare systems
- Ability to work collaboratively with internal and external stakeholders
- A vehicle and a valid Ontario Driver’s license are required
- Ability to manage coinciding demands
- Demonstrated flexibility and adaptability
- Experience working in a health care environment and/or working in a non-profit organization

What We Have to Offer You
The hiring range for this position is $71,506.50 to $85,722.00 per year, which reflects the expected starting salary based on qualifications and experience. Please note, this is not the full salary range for the position. Opportunities for growth within the full salary range are available in alignment with organizational policies and performance
