Job Description
  • What You Will Do
    • Assess clients holistically, evaluating physical, cognitive, emotional, and environmental factors that affect daily living
    • Develop personalized care plans in partnership with clients and families that reflect their goals and priorities
    • Implement evidence-based interventions to restore function, promote safety, and maximize independence
    • Support rehabilitation in areas such as mobility, self-care, home safety, cognitive retraining, and return-to-activity
    • Educate and empower clients, families, and caregivers with training and strategies for success at home and in the community
    • Promote independence and prevention through early intervention, proactive safety strategies, and self-management support
    • Assist with funding applications for adaptive equipment, assistive devices, and home modifications
    • Collaborate with case managers, health professionals, and community partners to deliver seamless client care
    • Advocate for clients by connecting them to resources, services, and supports that enhance quality of life
  • Qualification
    • Registration (in good standing) with the College of Occupational Therapists of Ontario
    • Strong clinical expertise with a focus on rehabilitation, safety, and independence
    • Excellent time management and organizational skills; ability to work independently in the community
    • Strong communication and teaching skills to inspire confidence in clients and families
    • Strong Critical thinking, problem-solving, and sound decision-making
    • Valid driver’s license (G2 or higher) and reliable vehicle
    • 2–5 years of clinical practice experience preferred
    • ADP Certification strongly preferred
    • Experience in community health is an asset
    • Bilingualism (French/English) is considered an asset

The Opportunity: Part-Time Personal Support Worker/Homemaker (0.9 FTE or 67.5 hours bi-weekly)

Job Description
  • What You Will Do:
    • Assist in the activities of daily living including eating, dressing, transferring, toileting, grooming, and bathing
    • Provide laundry of clothes and linens
    • Provide meal set-up and preparation
    • Provide cognitive, social, and compassionate support
    • Document reports for clients on their plan of care and any changes in behavior of daily routines
    • Maintain a clean, safe, home environment
    • Sweep, vacuum, sponge mop floors
    • Dust furniture
    • Clean bathrooms

  • What You Will Do
    • Deliver therapeutic exercise programs (individual and group) under the direction of the Physiotherapist
    • Support rehabilitation progress by monitoring client abilities and reporting changes to the Physiotherapist

Job Description
  • What You Will Do:

    As a Registered Nurse Hospice Cornwall, you will make a difference in the lives of your clients by completing relevant assessments and developing care plans based on those assessments with the client and their family. You will implement nursing strategies or interventions according to the care plan and perform technical nursing skills in accordance with nursing principles and
    Carefor’s policies and procedures.
    You will evaluate, document and describe client/family responses to the care plan, and revise the plan as required on an ongoing basis. You will report and discuss the status of your client and their care on a regular basis with other members of the health care team to ensure the most appropriate and relevant care is being administered.

  • Qualifications

At Carefor we are in ongoing need of Personal Support Workers to support our clients in their homes and in retirement settings. You will be providing them with care and support so they can stay at their own homes longer and more safely.

Job Description
  • What You Will Do:

    The Personal Support Worker is a critical role within our communities as it positively affects the lives of clients and ignites the warmth of human connection. You will be making a difference in the lives of your clients each day by providing personal care, delegated exercise programs, and activities of daily living (ADLs) as prescribed by each client’s individual care plan while collaborating with all members of the health care team to meet the needs of individuals to attain their optimal level of independence and well-being.

    You will make an impact on the lives of clients in the following ways:

    • Assist in the activities of daily living including eating, dressing, transferring, toileting, grooming, and bathing
    • Provide cognitive, social, and compassionate support
    • Document reports for clients on their plan of care and any changes in the behavior of daily routines

    To provide optimal service to our clients as a Part-Time Personal Support Worker, we require that you are capable of making a commitment to accommodate 37.5 hours bi weekly. Must be able to work on shift rotations days and evening and weekends, every second weekend.

    A standard schedule will be respected, subject to changes as required by program demand, changes in client visits and caseload. To support planning, we offer you a schedule 2 weeks in advance.

  • Qualifications
    • Certificate in Home Support Worker (Level II, III), Personal Support Worker or Developmental Service Worker program, Health Care Aide, Foreign-Trained BScN or Doctor, Paramedic Graduate, BScN Nursing Student, or second year RPN student.
    • National Academy of Careers Colleges Certificate, if applicable
    • Your ability to manage confidential information
    • Your ability to work independently
    • Your ability to communicate effectively in English, both written and verbal
    • You can provide a Vulnerable Sector check (obtained within the previous six months)
    • You are comfortable working in various social and economic environments
    • You can provide up to date immunizations for diphtheria, tetanus, polio, rubella and hepatitis
    • You are comfortable using a smartphone to communicate with clients and Carefor office staff by phone and email
    • You are comfortable using a smartphone for screening and security applications (apps)
    • Experience with Palliative Care is considered an asset

    Salary:  $16.55 per hour
