Job Description
  • What You Will Do:

    As a Registered Practical Nurse in Carefor’s Visit Nursing Program, you will make a difference in the lives of your clients in the community by utilizing your excellent communication skills to obtain relevant information from your clients and their families. You will implement nursing strategies or interventions according to the established care plan in consultation with the client and/or caregiver and the health care team. You will utilize adult learning principals and teaching materials to provide guidance to clients and their caregivers. You will incorporate your research findings into your daily clinical practice while demonstrating your exceptional time management and advance planning skills that will promote client-directed care, self-care and independence.

  • Qualifications

    In our fast-paced, responsive, and caring environment, we are looking for a self-starter who has the experience and ability to jump in on day one with energy and enthusiasm.

    The following items describe important experience and competency areas you have demonstrated in your current or previous roles in order to be successful in this role:

    • You are registered in good standing with the College of Nurses in Ontario (CNO)

The Opportunity: Registered Practical Nurse (RPN) – Temporary Full Time (1.0 FTE or 75 hours bi-weekly)

Under the supervision of the Care Team Supervisor, the Registered Practical Nurse (RPN) is responsible for the provision and management of cost effective, quality nursing care to residents of the Carefor Mackay Centre. The RPN will assume a leadership role by teaching, delegating, and providing guidance to different levels of workers within the care team. Care is provided within the framework of quality & risk management, Carefor policies and procedures, and the standards and scope of practice set by the College of Nurses of Ontario (CNO).

This is a temporary position that will end on September 18, 2025, or when the incumbent returns.

Job Description
  • What You Will Do:
    • Provides complete, relevant assessment of the resident and describes and/or documents finding according to documentation procedures.
    • In collaboration with the Manager or designate, prepares and updates the resident profile and care plan
    • Effectively and safely administers medication according to Carefor procedures and CNO standards of practice
    • Collaborates with other members of the health care team to ensure residents receive quality care
    • Identifies problems using a specified framework and in consultation with the Manager or designate and resident, selects and implements individualized strategies or interventions in order to focus care on resolving or managing the problem
    • Describes, evaluates and documents resident’s responses to care
    • Describes rationale for referrals and changes in Service Plan
    • Provides cost-effective, organized, outcome directed care on an ongoing basis
    • Identifies and utilizes tools and resources for work and manages these resources through regular monitoring of equipment etc. and reporting unusable equipment to supervisor or designate.
    • Delegates care appropriately
    • Meets with resident and family to orient them to the residence.
    • Seeks input from residents to assure that service meets expectations
    • Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
    • Functions in accordance with the standards of nursing practice from the College of Nurses of Ontario
    • Functions in accordance with the Carefor policies and procedures and the strategic direction of Carefor Pembroke-Renfrew County
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents
  • Qualifications
    • RPN Diploma from approved college
    • Valid registration with the College of Nurses of Ontario
    • Certificate in gerontology would be an asset.
    • Current CPR and First Aid
  • Experience
    • 3-5 years’ experience
    • Experience in a long term care or retirement facility an asset
    • Experience working with people who have dementia, Alzheimer’s Disease, and mental health illnesses
  • Skills
    • Observation and health assessment according to level of practice
    • Performs duties according to the RPN scope of practice
    • Health teaching facilitation
    • Leadership skills
    • Effective verbal and written communication skills
    • Develop individualized care plans and interventions as determined by the need of the resident.
    • Ability to problem solve and to adapt nursing to independent practice
    • Ability to work collaboratively as a member of an interdisciplinary team
    • Ability to work a variety of shifts within the care team schedule
    • Ability to work independently in a busy, fast paced environment and prioritize duties

The Opportunity: Registered Practical Nurse, Permanent

Carefor Health & Community Services is in search of a Registered Practical Nurse at Richmond Care Home, which is a unique retirement residence dedicated to the care of women is living with dementia. Richmond Care Home is situated in the Village of Richmond, Ontario, where we offer a small, home-like, supportive environment for up to 16 residents. A Registered Practical Nurse with Carefor is an integral part of the Carefor Health Team and is responsible for the provision and management of nursing care to individuals and/or families in the community, as well as teaching and delegating tasks to different members of the Carefor team.

Please note that Richmond Care Home is not accessible by public transit.

This residence is a secure bungalow in the heart of Richmond Ontario and is designed to help residents feel as though they are living in a home away from home. The living room, kitchen and dining area are the central hub of the residence with comfortable, familiar décor. Our highly trained staff are encouraged to wear regular clothes instead of scrubs or uniforms, this encourages our residents to feel even more at ease. The Richmond Care Home provides a level of care, compassion, friendship and love that makes this a place like no other.

Job Description
  • What You Will Do:

    Primary Responsibilities include:

    As the RPN at the home you will function as the leader of the resident care team. Utilizing your professional knowledge to assess clients, create care plans, administer medication and monitor/ evaluate the well being of the clients. In the absence of the manager you are in charge to ensure the residence comfort and well being and that policies and procedures are being followed.
