Job Description
  • What You Will Do:

    KEY RESPONSIBILITIES

    Clinical Practice

    The RPN provides service consistent with the standards established by Carefor and CNO. The nursing process is utilized: assessment, planning, implementation and evaluation.

    • Provides complete, relevant assessment of the client’s functional abilities, general condition and activities of daily living following pre-set procedures
    • Implements the care plan based on the nursing assessment in collaboration with the physician, PSW’s and supervisor and review quarterly
    • Implements nursing strategies or interventions according to the established care plan
    • Performs technical nursing skills at a safe level in accordance with scope of practice, nursing principles and Carefor policies and procedures
    • Utilizes appropriate interviewing and communication skills to obtain information from the client/family
    • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor standards (by exception)
    • Evaluates client’s responses to care plan for the achievement of realistic client-centered outcomes; revises plan as required
    • Demonstrates respect for client’s choices and decisions
    • Utilizes adult learning principles and established teaching materials to provide health teaching to clients, unregulated care providers and volunteers
    • Reports and discusses client’s status on a regular basis with other members of the health care team, including physician when necessary
    • Works closely with the care team (including Personal Support Workers (PSWs), cooks, recreation/activity coordinator and/or volunteers), providing them with direction, teaching and support
    • Orders and checks new medications from the pharmacy and lists the medications on the MAR sheets
    • Medication administration to clients as per CNO standards
    • Admissions and discharges of clients as required
    • Keep up to date on changes in the field of dementia care
    • Familiarity with the home’s fire, disaster and evacuation plans
    • Participate in providing personal care, activities of daily living, preparing and serving meals and snacks, light housekeeping, recreational/personal interest activities, as necessary.

     

    Leadership

    • Works collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for fellow nurses, PSWs, volunteers, and students.
    • Shares knowledge, assisting clients to make informed choices in determining health and service needs
    • Influences policy review and development in health care and standards of nursing practice through involvement in Carefor, professional organizations and/or community organizations
    • Help create and maintain a peaceful and home-like environment for guests

     

    Quality & Risk Management

    • Practices in accordance with the quality and risk management framework of Carefor
    • Articulates knowledge of components of quality and risk management
    • Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
    • Seeks input from clients and their family (POA) to assure that service meets expectations
    • Functions as an effective team member
    • Participates in decision-making and follows up to promote continuous quality improvement

     

    Professional Accountability

    • Functions in accordance with the Standards of Nursing Practice from the CNO
    • Functions in accordance with the Carefor policies, procedures and strategic direction
    • Functions in accordance with applicable legislation
    • Maintains competence through continuing education
    • Collaborates with management staff and colleagues in the evaluation of own practice

     

    Communication

    • Receives direction and assignments from the supervisor and/or manager by phone or in-person
    • Observes the client’s pattern of communication and relating and reports changes/concerns to the health care team and supervisor and/or manager
    • Relates to and shares information with other members of the clients health care team within and outside the organization in a professional and timely manner
    • Reports verbally to the physician/family any changes in the individual’s personal support needs necessitating a change in the care plan
    • Utilizes verbal and non-verbal communication skills effectively with the client
    • Communicates with the clients respectfully and according to their needs and situation/condition
    • Uses effective communication strategies to manage inappropriate, disruptive or aggressive behaviour
    • Consults with the supervisor and/or manager when unable to resolve difficulties with a client or team member or when requiring clarification about an issue
    • Uses interpersonal skills to build caring/helping relationships with clients
    • Acts as a team player, communicating effectively with all team members

     

    Daily Routine:

    • On arrival at work receive report from the departing staff and at the end of the shift give report to the arriving staff
    • Reads all relevant memos, care plans, and other written instructions or directions pertinent to that day’s shift
    • Enters all necessary information in the chart as required
    • Enters relevant information in the staff communication log as required.
    • Medication administration to clients as per CNO standards
    • Participate in providing personal care, activities of daily living, preparing and serving meals & snacks, light housekeeping, recreational/personal interest activities as necessary.

     

     

     

     

     

  • Qualifications

    Qualifications

    • Registered Practical Nurse
    • Current registration with the College of Nurses of Ontario
    • Current Standard First Aid Certificate
    • Current CPR level C Certificate
    • Nine (9) months to one (1) year of experience working with elderly clients with dementia

     

    Qualifications considered an asset

    • Experience working with Dementia clients preferred

    Salary: $45,318 – $59,494.50

    Flexible Language Requirement:

    • French not required but considered an asset

    Schedule:

    • 8 hour shifts, Days, Evenings and every other weekend

    Expected hours:30 hrs/biweekly or 0.4 FTE
