We are seeking dynamic individuals to join the Carefor Health & Community Services (Carefor) Board of Directors. As a Director on Carefor’s Board of Directors, you will be involved in guiding a long-standing organization to meet the diverse and evolving healthcare needs of our community.

About Us:

Carefor Health & Community Services, established in 1897 as the first branch of the Victorian Order of Nurses (VON), has a long and rich history of providing home healthcare services. Carefor is the largest locally-based not-for-profit homecare and community support services provider in Eastern Ontario, and one of the largest in the province. With over 1,300 employees and an annual operating budget of ~$70M, Carefor is unique in the region due to the comprehensive suite of homecare services offered including nursing, occupational therapy, social work, in-home physiotherapy support services, and a wide variety of community health services.

Carefor also owns and operates several residential care facilities offering retirement, respite care for clients living with dementia, specialized supportive housing, and palliative care hospice services.   • Commitment Overview

    Board Directors are elected for a three (3) year term with possibility for re-election. Board Directors are asked to sit on at least two (2) Board Committees. On average, Board Directors should expect to attend four to five (4-5) meetings a year for the Board and each Committee.

    As a Director, you will receive a comprehensive orientation and be assigned a mentor to guide your growth and development.

  • Desired Qualifications and Characteristics

    We are looking for energetic, skilled individuals to become part of a strong team as we build for the future. Preferred candidates bring experience with the health care sector or caregiving, a strong professional or business background, demonstrated leadership and time to contribute meaningfully to the organization. Areas of specific expertise are reviewed on an ongoing basis and matched with current needs. Knowledge of Carefor is an asset.

Under the supervision of the Care Team Supervisor, the Registered Practical Nurse (RPN) is responsible for the provision and management of cost effective, quality nursing care to residents of the Pembroke Civic Complex/Carefor Mackay Centre. The RPN will assume a leadership role by teaching, delegating, and providing guidance to different levels of workers within the care team. Care is provided within the framework of quality & risk management, Carefor policies and procedures, and the standards and scope of practice set by the College of Nurses of Ontario (CNO).

Job Description
  • What You Will Do:
    • Provides complete, relevant assessment of the resident and describes and/or documents finding according to documentation procedures.
    • In collaboration with the Manager or designate, prepares and updates the resident profile and care plan
    • Effectively and safely administers medication according to Carefor procedures and CNO standards of practice
    • Collaborates with other members of the health care team to ensure residents receive quality care
    • Identifies problems using a specified framework and in consultation with the Manager or designate and resident, selects and implements individualized strategies or interventions in order to focus care on resolving or managing the problem
    • Describes, evaluates and documents resident’s responses to care
    • Describes rationale for referrals and changes in Service Plan
    • Provides cost-effective, organized, outcome directed care on an ongoing basis
    • Identifies and utilizes tools and resources for work and manages these resources through regular monitoring of equipment etc. and reporting unusable equipment to supervisor or designate.
    • Delegates care appropriately
    • Meets with resident and family to orient them to the residence.
    • Seeks input from residents to assure that service meets expectations
    • Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
    • Functions in accordance with the standards of nursing practice from the College of Nurses of Ontario
    • Functions in accordance with the Carefor policies and procedures and the strategic direction of Carefor Pembroke-Renfrew County
    • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
    • Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
    • Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
    • Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents
  • Qualifications
    • RPN Diploma from approved college
    • Valid registration with the College of Nurses of Ontario
    • Certificate in gerontology would be an asset.
    • Current CPR and First Aid

    Job Types: Part Time (0.4 FTE or 30 hours bi-weekly)

    Salary: $24.58 per hour
