Carefor offers home care community services and retirement homes for people in Eastern Ontario

Services in your community to help you stay at home

You want to be able to continue to live in your home and Carefor’s community services help you do just that. We offer a variety of services which help you stay connected.

  • Companion Program

    Our Companion Program provides respite to caregivers by taking responsibility for a client’s practical, emotional, and/or social support.  Our companions can assist people with a variety of tasks, including light housekeeping, meal preparation, companionship, escorts to appointments, and more.

    There is a fee for service.

  • Bereavement Support

    Staff and volunteers support individuals, family, and friends following the death of a loved one, individually or in a group setting.

  • Family Resource Centre

    Staff and volunteers provide education and resources to help clients and families find information (for example, internet access to resources, library materials, workshops, and webinars) and one-on-one support services.

  • Personal Support Services

    If you need some help with your care, our trained Personal Support Workers can help.

    Some of their services include:

    • Personal Care: bathing, grooming, dressing, exercise and more.
    • Respite Care: Relief for caregivers, companionship and life enrichment activities.
    • Home Management: Light housekeeping and meal preparation

    Service is available 24 hours a day, seven days a week in blocks of time from 1 to 24 hours.

    There is a fee for service.

Assisted Living Services

If you need some extra help at home.

A Home and Community Care Support Services funded program, Carefor’s Assisted Living Services are community-based health care services that provide eligible seniors living in Pembroke with the assistance they need to allow you to continue living on your own for as long as possible.

There is no cost for these services.  Spaces are limited and there may be a waitlist.

Going Home Program

The Going Home Program helps seniors remain safely at home after leaving the hospital.

The program offers a basket of community support services, including frozen and hot meals, transportation, homemaking and personal support services. Services are free of charge within the first 10 business days when leaving the hospital. 

How do I get service?
  • Assessment

    Clients are assessed and referred to the Going Home Program by the Hospital Discharge Planner and/or by the Geriatric Emergency Management (GEM) Nurse in the Emergency Departments of our Champlain District hospitals.

  • Eligibility
    • Age: 65 years of age and older.
    • Mobility aligns with Champlain Community Transportation Collaborative criteria.
    • Services will not be duplicated either through Home and Community Care, new or existing services in the community, or paid services.
    • Clients living in Retirement Homes are eligible for transportation services only.
    • Clients with contact precautions are eligible dependent on the availability of PPE for client and driver (if necessary), and referral includes a description of the precaution. Intake reserves the ability to refuse the referral if deemed unable to safely service the client.

Helping you maintain function and choice in how you want to live.

Our experienced and trained rehabilitation team offer services to help you build and maintain independence offering you support in your home.

Our home care rehabilitation services in Ottawa include physiotherapy, occupational therapy, dietetics and social work support.

Friendly Visiting

Feel connected with your community.

The Friendly Visiting program offers one-on-one visits with a Carefor volunteer or team member for people who find it difficult to leave their home. In addition to making home-visits, our team of professionals are happy to lend an ear by offering one-on-one phone calls with seniors, so they maintain a strong personal connection to their community.

Respite & Short-Term Stays

You can access comfortable temporary accommodations at the Carefor Civic Complex and Carefor Mackay Centre, arranged with the level of support that you need.

Winter stays are ideal for people wishing to have a break from winter-time household responsibilities, for seniors requiring respite care or for those who wish to learn more about retirement living.

Short-term stays are offered to those recovering from a recent illness or surgery.

Your stay with us will include:

  • 24-hour staffing and basic assistance with personal care
  • housekeeping and laundry services
  • 3 nutritious meals per day, plus snacks
  • medication assistance
  • recreation programming

Community Volunteer Visiting

Helping you stay in your home longer.

Our compassionate and fully-trained volunteers offer companionship, practical assistance, and information to socially isolated or vulnerable clients in their home, in long-term care facilities, in retirement homes, and at the hospital.

Our services include:

Self-referral and referral from a family member or health professionals are accepted with the client’s permission.

There is no fee for these services. Donations are always appreciated!

It’s more than just a ride to an appointment.

Our non-urgent transportation program offers flexible and affordable transportation to help individuals unable to access other means of transportation to local and out of town medical appointments. But it’s more than just a ride, it’s a friendly smile along the way.
