Carling Adult Day Program

Come for the care, stay for the fun.

The Carling Day Program gives people living with dementia a chance to enjoy a fun day out with friends in a safe and supportive setting, while giving their loved ones a well-deserved break.

Information, supportive counseling and referrals to other community services and programs are also available.

Program details

When is it: Monday to Friday from 9:00 am – 3:00 pm

Cost: $25.00 per day.

How can I join: Program referrals are made by Home and Community Care Support Services Champlain (HCCSS), as well as health care professionals and other Community Support Services and programs.

Inquiries are always welcome by calling the program directly.

Transportation

Cost: We can arrange transportation for participants living within the program’s boundaries for $16.00 (return trip).

Pick up: Participants are usually picked up between 8:00 and 9:00am

Drop off: Between 2:45 and 3:30pm.

Staff can also assist with accessing community transportation services or para Transpo. A family member or designate must be there to meet the client at the end of the day.

Going Home Program

The Going Home Program helps seniors remain safely at home after leaving the hospital.

The program offers a basket of community support services, including frozen and hot meals, transportation, homemaking and personal support services. Services are free of charge within the first 10 business days when leaving the hospital. 

How do I get service?
  • Assessment

    Clients are assessed and referred to the Going Home Program by the Hospital Discharge Planner and/or by the Geriatric Emergency Management (GEM) Nurse in the Emergency Departments of our Champlain District hospitals.

  • Eligibility
    • Age: 65 years of age and older.
    • Mobility aligns with Champlain Community Transportation Collaborative criteria.
    • Services will not be duplicated either through Home and Community Care, new or existing services in the community, or paid services.
    • Clients living in Retirement Homes are eligible for transportation services only.
    • Clients with contact precautions are eligible dependent on the availability of PPE for client and driver (if necessary), and referral includes a description of the precaution. Intake reserves the ability to refuse the referral if deemed unable to safely service the client.

Homemaking

Homemaking Services help seniors stay at home longer.

The Program provides tailored light housekeeping services for seniors and physically disabled adults living at home. To qualify for the program, clients are assessed by a Carefor team member.  There is a minimal cost for the services, which may include housekeeping tasks like cleaning, laundry and changing linens.

Eligibility

Seniors over 65 years of age who meet the requirements based on the mandatory InterRai CHA health assessment.

*Note: There is currently a waitlist for this service. You will be contacted as soon as your referral can be reviewed.

Our home care rehabilitation services in Ottawa include physiotherapy, occupational therapy, dietetics and social work support.

Personal Support Services

Carefor Personal Support Workers are available 24/7 to support you, offering independence, connection and caring, whether at home, in a retirement home or assisted living.

We offer home care personal support services in Ottawa, Pembroke-Renfrew County and Eastern Counties that gives you and your family peace of mind.

Carefor also provides Client Care Workers as part of our Inner City Health Partnership with Shepherds of Good Hope and Ottawa Inner City Health to support people living with homelessness across Ottawa.

Let’s Get Together Club

A safe space for seniors with mental health challenges.

Participants are provided with opportunities to socialize, develop friendships, and engage in leisure, education and wellness activities in a safe, structured and supportive setting.

Information, supportive counseling, consultation with health professionals, and referrals to other community services and programs is also offered.

Program details

When is it: Mondays, Wednesdays & Fridays from 9:00-3:00.

Cost: $10.00 per day.

Who is it for: isolated older adults (60 plus) living with a mental illness such as: chronic depression, anxiety and bipolar disorder.

FINCH: Offered at the North-Stor Seniors Support Centre, our Adult Day Program provides supervised and supportive activities for seniors and people with specialized needs such as dementia and physical disabilities.
