The Hospice RPN is responsible for the provision and management of cost effective nursing care to the clients at Carefor Hospice Cornwall. Care is provided within the framework of quality & risk management and the standards and scope of practice set by Carefor and the College of Nurses of Ontario. The nurse is responsible to teach and delegate tasks to different levels of workers, including Personal Support Workers and volunteers. The nurse creates an environment of practical, psychosocial and spiritual support for the clients of the Carefor Hospice Cornwall.

Job Description

What You Will Do:
Clinical Practice: The Hospice RPN provides service consistent with the standards established by Carefor and CNO. The nursing process is utilized: assessment, planning, implementation and evaluation.
a) Provides complete, relevant assessment of the client, following pre-set procedures
b) Implements the care plan based on the nursing assessment in collaboration with the Hospice RN or physician
c) Implements nursing strategies or interventions according to the established care plan
d) Performs technical nursing skills at a safe level in accordance with scope of practice, nursing principles and Carefor policies and procedures
e) Describes, evaluates and documents client’s responses to the care plan utilizing the electronic record
f) Utilizes appropriate interviewing and communication skills to obtain information from the client/family
g) Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor standards
h) Evaluates client’s responses to care plan for the achievement of realistic client-centered outcomes. Revises plan as required
i) Demonstrates respect for client’s choices and decisions
j) Utilizes adult learning principles and established teaching materials to provide health teaching to clients, unregulated care providers and volunteers
k) Reports and discusses client’s status on a regular basis with other members of the health care team
l) Liaises with the Hospice staff.
m) Works closely with the PSW’s and volunteers, providing them with direction, teaching and support
n) Orders and checks new medications from the pharmacy and lists the medications on the MAR sheets
Leadership:
a) Works collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for PSW’s, volunteers and students.
b) Shares knowledge, assisting clients to make informed choices in determining health and service needs
c) Influences policy review and development in health care and standards of nursing practice through involvement in Carefor, professional organizations and/or community organizations
Quality & Risk Management:
a) Practices in accordance with the quality and risk management framework of Carefor.
b) Articulates knowledge of components of quality and risk management
c) Plans, implements, evaluates and adjusts care in accordance with customer focused expectations
d) Seeks input from clients to assure that service meets expectations
e) Participates in decision-making and follows up to promote continuous quality improvement
Professional Accountability:
a) Functions in accordance with the Standards of Nursing Practice from the CNO
b) Functions in accordance with Carefor policies, procedures and strategic direction
c) Functions in accordance with applicable legislation
d) Collaborates with management staff and colleagues in the evaluation of own practice
e) Maintains competence through continuing education
Health & Safety Accountability:
a) Functions and reports in compliance with the Occupational Health and Safety legislation & regulations, and Carefor policies and procedures
b) Participates in health and safety training including WHMIS, and applies this knowledge in the workplace
c) Wears or uses any/all protective equipment or clothing provided by Carefor and does not interfere with protective devices so as to change or negate their functions
d) Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker
e) Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard
f) Does not use or operate any equipment, machine, device or thing, or work in a manner that may endanger him/herself or any other worker
g) Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents
h) Documentation: Drafts and updates documents for internal use on procedures, processes and systems related to the incumbent’s responsibilities.

Qualifications
- Licensed as a Registered Practical Nurse in the province of Ontario
- Clinical practice experience mandatory
- Experience in community health preferred
- Palliative Care mandatory
- Excellent skills in assessment, risk identification and management
- Collaboration, consultation and counseling to resolve issues
- Enhanced chronic disease monitoring and management
- Health teaching
- Excellent verbal and written communication skills
- Interpersonal relationship building
- Ability to work with regulated and unregulated care providers and volunteers

Salary: $50,992.50- $59,494.50

- 8 hour shifts
Expected hours: 75hrs/biweekly
