SUMMARY OF POSITION

As a Foot Care Nurse, with Advanced Foot Care training, is responsible for providing a wide range of foot care services, either in the home, in a Carefor clinics, at the Perley Rideau Health Center or at our community foot care clinics. Foot care nurse will utilize a General & Diabetic Foot Assessment Tool which follows the Registered Nurses Association of Ontario Best Practice Guideline “Reducing Foot Complications for People with Diabetes”, on all new admission clients to Carefor.

Job Description: Registered Practical Nurse

Is responsible for the provision and management of quality, cost effective nursing care to individuals and/or families in the community/clinic settings. Care is provided within the framework of quality & risk management and the standards and scope of practice set by Carefor and the provincial regulatory body (Ontario College of Nurses) for the Registered Nurse/Registered Practical Nurse. Is responsible to teach and delegate tasks to different levels of workers and clients/families.

KEY RESPONSIBILITIES

1. Clinical Practice
a) Utilizes appropriate interviewing and communication skills to obtain information from the client/family
b) Provides complete, relevant assessment of the client/family and their environment utilizing established procedures
c) Describes and documents assessment findings utilizing established procedures
d) In collaboration with the health care team, contributes to and updates the clients care plan as necessary
e) Documents problem(s) identified using a specified framework and focuses care on appropriate problem areas utilizing established procedures
f) Selects and implements strategies or interventions from established care plans in consultation with the client and/or caregiver and health care team
g) Describes, evaluates and documents client’s/family’s responses to care utilizing established procedures
h) Provides rationale for and documents modifications to the care plan (eg. visit schedule)
i) Negotiates with client/family during delivery of care and respecting client’s choices and decisions
j) Utilizes adult learning principals and established teaching materials to provide health teaching to clients and families
k) Incorporates current research findings into clinical practice
l) Documents clearly, concisely, accurately and appropriately according to Carefor standards
2. Professional Accountability
a) Functions in accordance with the Standards of Nursing Practice from the appropriate provincial regulatory body (Ontario College of Nurses)
b) Functions in accordance with the Carefor policies, procedures and strategic direction of Carefor
c) Functions in accordance with applicable legislation
d) Maintains competence through continuing education and CNO Learning Plan.
e) Collaborates with management staff and colleagues in the evaluation of own practice
f) Adheres to Carefor’s professional code of conduct and dress code

3. Quality & Risk Management
a) Practices in accordance with the quality and risk management framework of Carefor
b) Articulates knowledge of components of quality and risk management
c) Plans, implements, evaluates and adjusts care in accordance with customer focused
expectations
d) Collaborates with district coordinator, as able, in minimizing travel between clients while
understanding that travel is inherent in community nursing.
e) Seeks input from clients to assure that service meets expectations
f) Functions as an effective team member
g) Participates in decision making and follows up to promote continuous quality
improvement
h) Respects all legislative standards and directives.
4. Leadership
a) Works collaboratively within a multidisciplinary team and acts as a clinical resource and
mentor for other care providers, staff and students
b) Shares knowledge, enabling clients, family and community to make informed choices in
determining health and service needs
c) Promotes involvement of client/family in determining health and service needs
d) Influences policy review and development in health care and standards of nursing
practice through involvement in Carefor professional organizations and/or community
organizations

QUALIFICATIONS, EXPERIENCE, CREDENTIALS

 Registration as a Registered Practical Nurse in the Ontario College of Nurses
 For positions requiring expertise in a specific field of clinical practice additional
degrees/certificates may be preferred or required
 Clinical practice experience preferred
 Experience in community health preferred

Salary: $25.62-$28.67 per hour

Qualifications:

  • Currently registered and in good standing with the College of Nurses of Ontario
  • Certification in advanced foot care required
  • Experience in foot care clinics required
  • Previous community experience will be considered an asset
  • Ability to organize work and implement a plan of care
  • Possession of a valid driver’s license (G2 level)
  • Use of a reliable vehicle that is insured for business use
  • Good knowledge of IT technology i.e. laptop, cellphones
  • Bilingualism (French/English) considered an asset
